DESCRIPTION
Persons Involved: C.C. Dr. Y

● Patient C.C. initially sought consultation to the hospital for electrolyte


imbalance with an admitting orders from Dr. Y.

● After they have accomplished the necessary forms including health


declaration form stating that they have no flu-like symptoms. They were
accepted in the ER, but upon assessment to overt symptoms such as cough,
and shortness of breath. These were referred to ROD and the patient was
transferred to triage; treatment was also initiated.

● Policy and protocol dictates that they need to be tested for COVID-19.
Initially they refused the test but it was explained to them the process of
admission. They agreed to it eventually.

● As the result of the RT-PCR was disclosed, the patient confessed that they
are already confirmed COVID positive and supposedly, on quarantine.

● Contact tracing, testing and isolation was done to the exposed staff and
disinfection was also performed in the ER

• Thorough investigation and proper coordination with the
concerned authorities was done.

• Proved that the patient indeed violated Republic Act 11332


and a case was filed against them

• The patient still got admitted and proper treatment was


provided
