 Action required after receiving the self test kits:
• Upon receiving the test kit, you can distribute to the employees immediately.
• Employee to perform the self test between 24th to 28th August 2021.
• After performed the self test, employee needs to take photo of the test kit result as an
evidence record keeping (Write WWID & date on the test kit – see pic on the right)
• The team will design a portal for the employees & contract workers (CW) to upload the
result accordingly. Targeted to be release on 24th August 2021.
• Disposal of the self test kit is to be put inside the disposal bag given inside the test kit. Tied 1XXXXXXX

up properly and dispose into the normal waste bin as general waste.
• Self-test results:
• If the result is positive, contact OHN immediately for further instructions & upload the result to your
MySejahtera app
• If the result is negative, also upload to your MySejahtera app. All test result must be uploaded every 2 weeks
upon completion of self-test
• If the result is invalid, please re-do the self testing
• All self-test need to be conducted every 2 weeks before coming on-site (refer to the
communications by your BU)
