FIRE- ACCIDENT.
Incident Description

 On Sunday; 6:45 pm, HDPE pipes got fire

 Local public observed the fire & informed the fire Service.

 Fire tender reached the location & firemen were extinguishing the fire.

 After getting the information site team reached the location to oversee the
emergency response operation.

 Within 2 hours fire was totally brought under control by Fire department, no
personnel injuries were reported.

Causes of Incident
 Pipe was stacked below HT
power line.

 Due to short circuit, flame got


initiated, wire cut & fell down
upon the HDPE pipes &
nearby combustible material

 Combustible items like papers,


polythene, plastics, half burn
wooden items were thrown by
public near to pipes.

 Combustible items got


ignition source from
electrical short circuit & fire
began and lead to transfer of
heat to HDPE pipe.

 Fire got expanded & reached


to the other pipes present in
the vicinity.

Root Causes of Incident

Immediate Actions taken-

 Called on 101 emergency number for Fire


Service support.

 Local police was informed.

 Public was alerted & kept away from


location of incident, restricted from
unauthorized entry.

 All nearby combustible materials were


removed (starvation) to prevent further
escalation of fire.

Lesson learned
 Manufacturer’s guidelines of stacking &
storage to be followed.

 Store the pipe immediately before the


work begin as practically as possible.

 Storage of material and combustible


needs to be selected in such a way that
the source of ignition and its presence
can be avoided as practical as possible.

 The area around the HDPE pipes should


be neat and clean & it should be ensured
that no combustible items present in
vicinity.

 If possible, HDPE pipes should be


placed inside any closed boundary
premises, where security can be ensured.
