The process now is there shall only be 1 authorization CAB and 1 informational CAB.
E.g. Your change will No longer have to go to the Thai and Holden for Authorization, it will be
authorized in one and be information for the Regional CAB.
Please note, that SAP, M&Q and some other business areas have their own scoring matrix.

1.1 Change Details and Implementation Plan


Its all about details.
Please don’t just write a few lines.
Elaborate!

1.2 Identified Risks and Mitigation Strategies


Please give this area some thought.
Many a time the at CAB, this area
is left blank and risks are
identified by GM. This just
destroys your credibility as change
champion.

1.3 Backout Plan


Again, details!
And always ensure the timing you
have entered is logical and
sufficient.

Notification…who do you have to notify
regarding your change?
Have you done so?
If so, who? What is the contact number?

1.5 Test Plan


Before implementation.
If you have done so on a test box,
please
write down when, what box and if it was
successful.

Like a verification plan.

1.7 Quality Requirements – Why?

Too many mistakes.


Lack of detail forces the authorizers to interrogate and waste time on your change.
The questions they ask cannot be answered…shows HP hasn’t planned the change
properly.
Too many emergencies have created a reputation of tardiness.
Bringing changes to a CAB and not having approvals.

Please socialize your change with the appropriate
person and have it approved.

[when]” in the worklog.

Implementation Workorder

Note – Description : GM-GSC36-AP-Implementation Workorder


This workorder is used to detail the activities that take place during the implementation of
the change.
3 actions are required here by the implementor:
Fill in the actual start and finish time (Timing TAB)
Fill in the completion details (Closure TAB)
Close the workorder (Main screen)
Note: This workorder will be assigned to you workgroup by Dev or Kevin after it is approved
in the CAB

Go to the Timing Tab and please fill in the actual start and finish times

Under the Closure TAB:


Fill up an appropriate closure code as well
as completion details

Finally, close the workorder.


Choose close under the status
drop down box.
Enter “Workorder closed by [your
name] on [when]” in the worklog.
Then save and close

This is the last step in the workorder


cycle.
It is after completion of the
implementation workorder that
Kevin or myself close the change
ticket.
Summary – Using Workorders
If you are a TAB approver:
Use the approval workorder for TAB approvers
Fill in the Approval TAB
Fill in the Closure TAB
Set the workorder status to Close and hit “Save and Close”.

If you are implementing the change:


Use the implementation workorder
Fill in the Timing TAB
Fill in the Closure TAB
Set the workorder status to Close and hit “Save and Close”.

Closing Changes
We have to close changes as soon as we can, GM monitors this quite closely.
So please try not to forget completing the Implementation Workorder
Approvals
Please attach them to the OVSD ticket.
Try not to do it 5 mins before the CAB, we’re always glad as long as we get
approvals, but we now have to upload approvals to a website called quickplace.
Rescheduling Changes
Don’t leave your changes as they are if they were not executed.
If they are to be cancelled, cancel them, if not reschedule them and put in future dates.
Emergencies/Break Fixes
Paperwork can come later.
All you need is GM approval to carry out the change.
Please fill up the emergency change form if you create emergency changes and send it
back to Dev and Kevin.
