Consequences: Foot fracture

What happened:. The operator pulls the drawer


of the rack to the position that enables him to
take the mold.
This time the drawer blocker did not work, the
drawer and the mold fell down on a vertical
position on the floor, causing a minor fracture on
his right foot

What did we learn?


=> lack of visual (every 6 months) and legal
inspections (every year) for safety blockers
=> lack of controls on mold weights

Improvements on site
=> High runner molds stored on the floor,
=> Heavy molds only in the first floor,
=> Display of mold weight on each mold
=> Regular inspections
