SYMPTOM ASSESMENT

Case

On November 15, 2021, a female patient named Mrs. D, 45


years old, came to the clinic with complaints of dizziness,
frequent urination and blurred vision. After checking blood
sugar, Mrs. D suffers from Diabetes Mellitus with current blood
glucose levels of 420 mg/dL, blood pressure 170/100 mmHg.
History of medication glukodex 2 times a day, for hypertension
ditiazem 30 mg 3 times a day, captopril 25 mg 3 times a day, and
aspirin 100 mg once a day.
Definition

Diabetes mellitus is a disease caused by insulin deficiency or


reduced effectiveness of insulin. It is characterized by
hyperglycemia or an increase in glucose levels beyond normal.
Diabetes is characterized by persistent hyperglycemia. Insulin is
a hormone produced by the pancreas. The pancreas secretes
insulin into the bloodstream. Insulin helps glucose to enter the
cells. Insulin lowers the amount of sugar in the blood.
Symptom

 Feeling thirsty quickly (polydipsia) and urinating frequently (polyuria).


 Idipsia arises because most of the water in the cells is drawn into the blood (which contains
high amounts of glucose) due to differences in osmotic pressure. As a result, the cells are
dehydrated.
 Iuria arises because there is too much water in the blood vessels that needs to be
removed.
 Frequent extreme hunger (polyphagia).
 Ifagia arises because most of our body's cells are starving (not getting the food they need),
because glucose as a result of the breakdown of the food we eat does not enter the cells.
Management

 Make a commitment to be able to maintain glucose levels in the blood.


Take the medication as recommended by the doctor. Consume healthy
foods and do physical activity every day to maintain body condition.
 Consult an ophthalmologist regularly every 6 months to see possible
symptoms of retinal damage, cataracts, and glaucoma.
 Supports the immune system. Elevated blood sugar levels can weaken
the immune system. This can cause the body to be easily infected with
bacteria and viruses.
Complications

 Having problems with the heart and blood vessels.


 Have damage to the nerves (neuropathy). Neuropathy
characterized by frequent itching for no reason.
 Damage to the kidneys (nephropathy).

Mrs. D : I'm afraid of getting sick, nurses

checking blood sugar, now I'm taking action mom's blood pressure first i will put a cuff on your arm,
then i will start doing the checkup, ma'am excuse me please roll up the sleeves ok?

Nurse : ok mom after I did the examination how are you feeling now?
Mrs. D : My feelings are getting better, ladies, just a little while at the blood sugar check
Nurse : Okay, that's how the examination is to get the mother's blood for checking her blood sugar,
Mrs. D : oh that's right, ners
Nurse : good mother from the examination that I did and found that the mother's blood sugar was 420
where this had exceeded the mother's normal limit, where the normal blood sugar was 90... and the
mother's blood pressure was 170/100 which was already exceeding the mother's normal limit, So my
mother recommends reducing excessive sugar consumption, such as rice that is too hot, reduce sugar in
the addition of your mother's diet and also for her blood pressure, you can reduce a little salt and you can
also exercise regularly for at least 30 minutes a day which is done 3-4 in a week mother, where exercise
is very important in maternal health

Nurse : Ok mom and this is the medicine you got
Mrs. D : Okay, ners
Nurse : ok my mother has finished doing the action, did you have any questions, right?
Mrs. D : No ners
