ORIENTATION
OJT Modes
• Schools and departments can use different teaching and
learning activities for OJT course: virtual internship,
mentorship and apprenticeship; company-based
research, work-from-home OJT; teacher-assigned
activities; Coursera/LinkedIn certification; problem-
based activities, or project-based activities.
1. VIRTUAL INTERNSHIP PROGRAM (VIP)

2. ALTERNATIVE LEARNING ACTIVITIES (ALA)
• problem-based activities, or project-based activities
VIP - COMPANIES
• ICEP a shall seek the confirmation of partner
companies to offer virtual internship program for the
3rd Qtr. The list of companies shall be provided to
the schools for proper dissemination.
• It is advisable that the VIP will still be under an official
partner company. However, in the event that the
student will choose to render his/her VIP in a non-
partner company, the usual process of coordinating
with ICEP for company accreditation and MOA
processing shall take place.

• OJT course advisers shall submit the proposed
alternative learning activities with the
corresponding credit hours to their respective
Deans and/or Program Chairs for their approval
• The student must comply with the requirements
indicated in the ALA form as approved by their
respective OJT course coordinator

• ERF FORM
• Provide email address where to address the letter
• Endorsement Letter to be sent directly to Company
• To be sent by Ma’am Sally

PROCESS
1. Identify if ALA or VIP
2. If VIP
a. Check company if with partnership with MAPUA
a. If not, submit COMPANY PROFILE and ERF
b. Fill up and Submit ERF
c. School will send Endorsement Letter to Company
d. Company will provide Acceptance Letter
e. Student will send Annex A and Annex B to company for
their signature
f. Submit Acceptance Letter and signed Annex A & B to
Course Adviser for signature and Dean’s signature
g. Course Adviser will send the documents to ICEP
h. Start OJT
PROCESS cont..
1. if ALA
a. Will be assigned to specific department/faculty for a project
b. Submit acceptance form to Course adviser
c. Forward Annex C document to assigned supervisor for signature and tasks and
deliverables.
d. Start OJT
2. Submit DAR (signed by supervisor)
3. After 244 hours, request for Certificate of Partial Completion from the
Company/Mapua Department
6. Submit CPC to Course Coordinator with the name and email address of
the supervisor
7. Grade form will be sent to the email address provided by the supervisor
8. Supervisor will return the email to SOIT for recording
9. Student submit all requirements to Blackboard Course for grading.
10.Incomplete submission merits incomplete grade
Note: Submission of project proposal should be done within the duration of
the course.

• Company Rating 50%
• Documents 40%
• Others 10%
(subject to coordinator’s preference)

Course
• DAR 1 (one file in pdf format)
• Project Proposal
• Certificate of Partial Completion (also to be sent to
course adviser’s email together with the email
address of the supervisor)
• Annex A and Annex B for VIP or Annex C for ALA
