HOSPITAL
• Ahalia Foundation Eye Hospital is the only JCI accredited eye hospital in
south India to win the prestigious Joint Commission International (JCI)
• Ahalia eye hospital was founded in 1984
• Also got ISO: 9001:2008 accreditations for quality of services,
infrastructure and service delivery.
• It have 250 staffs and 19 regional Centres across kerala
PERFORMANCE APPRAISAL
• Performance appraisal is the process of obtaining, analyzing and recording
information about the relative worth of an employee.
• The focus of the performance appraisal is measuring and improving the
actual performance of the employee and also the future potential of the
employee. Its aim is to measure what an employee do

To identify the services provided by various departments in the hospital.

Performance Appraisal Process in AFEH
• It is done by Head of department (HOD) of each department
• Usually done before 3 months of employee contract expiry
• And the report of performance Appraisal is submitted to HR department
• HR department verify the report and give increment according to the
performance of employees
• These report is send to the MD & Chairman for signing and verification
DAYWISE INTERNSHIP REPORT

Day 1:
File management
1. Personnel file: it includes all the details of the employees who are working
there. Also keep the resigned employee’s file for 5 years as in the norms of
Labor Act, helps further reference in future.
Education certificate, CV ,Experience certificate, Interview assessment form,
Police clearance certificate. etc
2. Indexation

Files are arranged in the store on the basis of index

Day 2

BLS – Basic life support
• Train the employee for handling the basic life emergency situation.
• Eg. CPR, choking, fire & safety, using of fire extinguishers (internal and external training. External
training conducted by kanjikode fire station)

• File Management
Boxing the resigned employees file
Day – 4
• Indexing the files and mark attendance of employees
Day – 5
• Filed new joinees Details in the personal file
Day -6
• Visited materials department
• All the materials needed to the hospital are kept in materials department. They store a
bulk amount of materials in the store and supply it to the department according to the
needs of the department. For purchase the materials from the material department, other
departments need to submit purchase intent. Then they supply materials to the
departments in accordance with the purchase intent.
Procedure of purchasing
1. Calling suppliers
2. Confirming rates and getting approval from MM
3. Releasing purchase order to suppliers
4. Follow up and arranging delivery
Day – 7
Visited Front office

• Identify the patient
•  Capture patient’s photo 
• Fill the registration form with ID proof 
• Equally distribute according to doctor wise (new patient) 
• Sign patient consent form
• Sent the patient to the doctor OPD
Day - 8
MRD: Medical Recording Department
• MRD kept all the files related to the patients for more than 5 years by
following the norms of JCI
• MLC files are kept separately. MLC (medico legal case) are made only
when there is any accidents which involves legal formalities. It cannot be
destroyed, even if the time passes. First it kept in the reception and then its
handover to MRD.

Findings
• Reception is the busiest department in a hospital which really attracts patient
with their customer service.
• Feedback and proper guidelines should be given to employees by their HOD to
encourage them.
• Marketing department is another main department working effectively for the
growth of the hospital.
• Most of the records are kept in written format
• Employees are given timely performance appraisal and training.
