CASE SER
 A 45-year-old with a history of asthma goes to the ER his states
was he has increased shortness of breath and chest tightness. The
patient he was complains of a cold and flu with sever hadchah. He
went to the neighborhood pharmacy and the pharmacist there gave
him a cold and flu prepration and fevadol plus.
 after three days patient satat not improve , he go to other
pharmacy and tell pharmacist he feel hadchah and fever more than
one week , so the pharmacist give him diclofenac sodium+ the
previous medcation
 He complains that After he using the drug, he has short breath
 Also he had stage 1 hypertension , His BP was 130/75 and 131/76 mm Hg
last year during an employee health screening at work, A.’s father had
hypertension and asthma. Mr A. smokes one pack per day of cigarettes and
thinks his short birth because of the medication. He does not doning any
regular exercise

CASE SER

 Physical examination shows he is 175 cm tall, weighs 108


kg (body mass index [BMI], 35.2 kg/m2), BP is 137/79 mm
heart rate is 80 beats/minute., his oxsogen rate was 80%
 The ER Dr notes scattered expira tory wheezes
throughout his upper and middle lung fields and a
decreased peak meter flow, The other physical
examination findings are essentially normal.

CASE SER
 The current therapeutic regimen for Mr A includes
propanolo – 40 mg (adrenal) every 12 h, fevadol plus
2 tablet TID ,dicofenaic sodum 75mg BID ,Colde and
ful preparation TID.

SUBJECTIVE DATA

1. A 45-year-old man
2. history of asthma , stage 1 hypertension
3. His BP was 144/84 and 146/86 mm Hg
4. A.’s father had hypertension and asthma
5. smokes one pack per day of cigarettes
6. No regular exercise

OBJECTIVE
 
•Height: 1.75 m Weight: 130 kg. BMI: 35 Temp: 98.8 (oral) HR:
80 O2R: 70 BP: 137/79.
•Physical examination
Inspection
Generally, Client in distress, on oxygen. Patient alert, oriented
to time place and person.
 No abnormality detected. The patient has peripheral cyanosis

OBJECTIVE

• Auscultation: S1 and S2 sound heard, no crackles,


wheezes present. No heart murmurs. Bowel sounds present
• Palpation: no tenderness on the abdomen. No swelling
• Percussion. Resonance on the chest and abdomen. All
the other body systems are doing well.

OBJECTIVE

• Investigations
Monitor vital signs 2 hourly, carry out
sputum and blood test to check for signs
of T.B and any infection, measure oxygen
saturation levels by pulse oximetry,
measure ABGs, and Pulmonary function
tests.
