INTERDISCIPLINARY CARE
> An interdisciplinary approach involves team members
from different disciplines working collaboratively, with a
common purpose, to set goals, make decisions and share
resources and responsibilities.
> A team of clinicians from different disciplines, together
with the patient, undertakes assessment, diagnosis,
intervention, goal-setting and the creation of a care plan.
The patient, their family and carers are involved in any
discussions about their condition, prognosis and care plan.
ACLAN
> INTERDISCIPLINARY vs MULTIDISCIPLINARY

In contrast, a multidisciplinary approach involves team

members working independently to create discipline-specific
care plans that are implemented simultaneously, but without
explicit regard to their interaction

IMPORTANCE OF INTERDISCIPLINARY CARE
> a greater proportion of complicated cases require an
interdisciplinary approach in order to deal with complex
multimorbidity, social and psychological issues.
> best possible outcomes are achieved through a
consultative, collaborative approach to care
> can help avoid risk averse thinking by weighing up the
risk against benefits for the patient.
> can improve patient outcomes, healthcare processes and
levels of satisfaction.
> It can also reduce length of stay and avoid duplication of
assessments, leading to more comprehensive and holistic MALAYAO
records of care.
ELEMENTS OF SUCCESSFUL INTERDISCIPLINARY
APPROACH
> LEADERSHIP - Positive leadership and management give
clear direction and vision for the team through promoting
an atmosphere of trust where contributions are valued and
consensus is fostered.
> PERSON-CENTRED PRACTICE - Involving the patient
in all aspects of their care empowers them to speak up and
contribute to decision-making.
> TEAMWORK - Ensure team members have clear goals,
and an understanding of their shared roles and
responsibilities within the team structure. UNLAYAO
Recognise the overlap in knowledge and expertise of staff
from different disciplines.
PHARMACISTS ROLE IN
INTERDISCIPLINARY CARE

Pharmacists integrated into interdisciplinary

Primary Care Teams globally demonstrated their
significant role in many direct patient care
activities, including medication management,
identifying adverse or incorrect medication usage,
counselling on medications, and effectively
optimizing a patient’s understanding of their own ACLAN

medication regimens to enhance overall quality of

life.

VALUE OF PHARMACISTS INTEGRATION
INTO INTERDISCIPLINARY CARE
According to a study in Canada, the benefits are:
appropriate medication use,
hypertension control,
medication consultations,
improved prescribing,
reduced healthcare utilization and medication
costs, and improvement in the management MALAYAO
of chronic conditions
COLLABORATION WITH PHYSICIANS
Pharmacists are expected to provide physicians
with:
reliable drug information,
optimize medication prescribing,
improve clinical documentation, services, and
recommendations,
and enhance patient care

COLLABORATION WITH OTHER
PROFESSIONALS
At Women’s College Hospital in Toronto,
collaboration between a clinical pharmacist, a
pharmacy resident, and physicians, allowed the
interdisciplinary team to successfully meet their
deprescribing goal, aligned with the World Health
Organization’s call for action to decrease avoidable
medication-related harm.
In a study in California, 90% of physicians noted ACLAN
improved medication management of their patients
and 93% of respondents recognized the pharmacist’s
PERSPECTIVES FROM PATIENTS
Patients and caregivers communicated the benefits
of having a comprehensive medication review in an
interdisciplinary setting.
This includes a decrease in inappropriate
medication use, optimization of hypertension and
diabetes medication regimens, as well as
minimization of antipsychotic use for the
behavioral and psychiatric symptoms of dementia
JOSE
Pharmacists in Ontario are now formally funded by
the public healthcare system to be members of the
interdisciplinary healthcare team.

Pharmacists within such teams are uniquely

positioned to facilitate the expansion of the
pharmacist’s scope of practice through a
collaborative care model that leverages, integrates,
and transforms the medication expertise of
pharmacists into a reliable asset and resource for MALAYAO

physicians.
