PROCESS
The nursing process is defined as a systematic,
rational method of planning that guides all
nursing actions in delivering holistic and
patient-focused care. The nursing process is a
form of scientific reasoning and requires the
nurse's critical thinking to provide the best care
possible to the client.

The nursing process is a systematic problem-
solving approach used to identify, prevent and
treat actual or potential health problems and
promote wellness.
For example, a nurse's assessment of a hospitalized
patient in pain includes not only the physical causes
and manifestations of pain, but the patient's response
—an inability to get out of bed, refusal to eat,
withdrawal from family members, anger directed at
hospital staff, fear, or request for more pain
mediation.
Diagnosis. This phase in the nursing process is one of
the most important. We must consider all external
factors of the patient (environmental, socioeconomic,
and physiological etc.) when developing a diagnosis,
which can be challenging at times.
