COOPERATIVE
REGISTRATION
PROCESS
1. RESERVE the proposed cooperative name online at www.cda.gov.ph

Proposed cooperative name:

a. Should include the Type and the word “cooperative”


b. Should NOT use the word:
i. Development
ii. Integrated
iii. Incorporated

c. Should NOT be identical, deceptively or confusingly similar to any existing


coop, contrary to public policy, morals or existing laws

d. Should not be written in all capital letters except for the acronym

2. Pay name reservation fee


3. Prepare the Economic Survey

A statement that describes, among others, the:

a. Purpose and economic projection;


b. Staffing pattern which shall include a Bookkeeper;
c. Area of operation; and
d. Size of membership.
4. Prepare the
Cooperative Bylaws

All 4 copies must be original and signed by all


members at the adoption page
5. Prepare the ARTICLES OF
COOPERATION.

All 4 copies must be original, duly notarized and


signed by all cooperators on each and every page
6. Secure Surety Bond of Accountable Officers

• Chairman
• Treasurer and Cashier
• Manager
• Warehouseman
• Loan/deposit collectors
• Signatories of checks

(Note: The adequacy of Bond should be determine by


the Board of Directors)
7. Execute Treasurer’s Affidavit

A Sworn Statements of the TREASURER elected showing


that at least 25% of the authorized share capital has been
subscribed and at least 25% of the total subscription has
been paid prior to registration.

Provided, That in no case shall the paid ­up share capital be


less than P15,000.00.
8. Submit the documentary
requirements

1. Four (4) Copies;


2. all Original;
3. Printed in long bond paper;
4. Bookman old style, size 12;
5. May be written in English, Filipino or
other dialect with appropriate English
translation.
