Important Reminders during

2022 Onsite Annual Physical Examination

1. As a health protocol, if you have an illness or any of the flu-like
symptoms, do not report for the onsite APE, but instead, you will just
be endorsed to SFCMC within 1 week for the in-house APE. You should be
asymptomatic or has recovered from your sickness.
2. Bring or wear plain shirt only as much as possible (no buttons or
any metal design). This is to prevent changing of your clothes or uniform
before x-ray & avoid cross contamination of wearing x-ray gowns.
3. Please wear face mask upon entry to MHECO Warehouse. You will
not be accomodated for APE without proper wearing of face mask.
4. Specimen containers for urine & stool will be distributed to the
department coordinator 1-2 days before the activity.

5. Bring your own black or blue ballpen, then get a Physical Exam
Form to Nurse April or Guard on duty upon arrival. Submit the
accomplished P.E. form to the SFCMC staff at the Registration Area (beside
Electrical & Instrumentation work area).
6. Please observe strict physical distancing.
7. Nurse April will notify the employee regarding availability of the APE
result and will request the said employee to visit her for APE discussion.
Note: APE compliance & discussion of results will be strictly
monitored.
8. In case of repeat examination, please ensure to complete the
requirement as advised by the OH Nurse.
