PERMIT TO WORK

Key Requirements

A Permit to Work (PTW) describes what you shall do to execute work in a safe

manner. All non routine* and high risk activities shall be executed under permit to

work issued by a competent personnel.

• The Permit Receiver and Permit Issuer shall have valid PTW training credentials**.

• The Work Permit shall be signed off and issued by approved Permit Issuer having

knowledge of the conditions under which the work will be carried out in his area, the

associated risks and control of simultaneous activities (SIMOPS).


• The permit receiver shall be competent in understanding the job being performed, it’s

intrinsic hazards and how to best control these hazards.

• The certificates mandated in the PTWs (Radiography Work Certificate, Gas Test etc.)

shall be available and signed off before start of work.

• The permit receiver shall explain to the work crew the PTW requirements and

conditions in the HIP discussion session.

• The workplace shall be inspected by the permit receiver& HSE Representative before

the start of each shift to confirm that it is safe to start work and to revalidate the PTW

conditions.

• Work Permit shall be reassessed and revalidated when the work scope or working

conditions changes / Shift change

• At the end of the shift/day, the permit receiver shall confirm that the work is

completed and safe conditions are reestablished or a proper shift handed over is

done before leaving the work site.

Use this checklist to verify compliance with the Critical Risk Protocol for Permit to

Work.

Is the permit signed off by an Approved Permit Issuer and received by an

approved permit receiver

Does the work permit define the scope of work?

Are the hazards and risks associated with the scope of work assessed and

documented along the PTW ?

Are the control measures mentioned in the PTW to eliminate or mitigate the

associated hazards implemented?

Are the associated checks and certificates required as per PTW completed/signed

off/made available before start of work ?

Are the work permits interlinked, if SIMOPS is involved?

Are the PTW requirements and conditions communicated to all parties involved

in the scope of work (including SIMOPS, as applicable) through HIP talk?

Has the permit receiver done a joint inspection along with the HSE representative

before start of work and has the HSE representative endorsed the PTW

conditions?
Are workers empowered to use Stop Work Authority if work conditions

prescribed in the permit are not met?

Is return to normal operations considered as part of PTW document and

adequately controlled ?
