FINAL SEMESTER Project: Objectives
/1/ Understanding a “real world” situation:
• Investigating an event/occurrence happened in real world of Airlines/MRO (or related organizations in
Aviation)
• Catching quickly and precisely critical information of the event, helping to analyze “the error”

/2/ Understanding root causes of problems (8 points)


• Understanding the elements/components contributing to “the error” (using SHEL/SCHEL models)
• Understanding how and why the events could happen (by using SWISS CHEESE models)
• Understanding how data were collected (investigation processes..)
• Suggesting/understanding improving solutions for future events (Company, Departments, Team/peoples
involved)

/3/ Open questions (2 points): How do you think a strong “Safety culture” would improve safety level of an
organization (such as an airline or an MRO)? Clarify your explanation or ideas by using the Case-Study
assigned.

/4/ Improving skills for learners:


• Teamwork
• Researching the relevant information/data relating to an aircraft incident/accident
• Writing a report by structuring relevant data to explain real event(s) happened in aircraft maintenance field

1) [1d] Statistics on studied events: How many similar events/occurrences worldwide (& In
Vietnam)? Main causes ?
2) [1d] On a most recent event/incident (or assigned event / with a cited Reference): Describe
the incident (as detailed as possible)  Where (Places/Airports/Routes); When (Time, Flight
phases,..); How (short describe the incident…), Consequences
3) [2d] Understanding the Error: Determine components of SHEL/SCHEL models ? Possible
breakdown or mismatch among the components ? Type of error(s) ?
4) [2d] Understanding How & Why: Analyzing by using Swiss Cheese model; How data were
collected? …
5) [2d] Answering “Open Questions”
6) [2d] Solutions (for improvement) ?  Authorities’, Manufacturer’s, Airlines’ , MRO/related
organizations, Departments, People/individuals,…
Report requirements:
1/ Clearly answer the above mentioned questions – including analyzing as deeply as possible
2/ Max 20 pages: max 8 pages of main report + max 12 pages of appendices (data / OEM’s articles
or official documents)
3/ Deadline: 5 days
4/ Submitting form of report:
+ Only: Word or PDF file (COVER+MAIN BODY TEXT)
+ Max: 2 files, 20 MB max
