RATIONALE

In response to the resumption of Face to Face Classes, the Dr. Filemon


C. Aguilar Memorial College of Las Pinas (DFCAMCLP) will conduct the
Internship Program as part of the Bachelor of Science in Business
Administration, Bachelor of Science in Accountancy, and the Bachelor
of Science in Information Systems curriculum for the school year 2021-
2022.
The College shall implement the On-the-Job Training program in
accordance with the guidelines outlined below.

YEAR 2021-2022
First Semester School Year 2022-2023
A. Preparation of the list of HTE partners and Linkages
B. Preparation of loading assignment of OJT coordinators
C. Preparation of OJT forms and documents that would include:
• • HTE and College Memorandum of agreement
• • Parent Guardian Letter of Consent
• • Endorsement Letter
• • Daily Time Record
• • Performance Evaluation Monitoring form
• • Practicum Mentoring report form
D. Orientation of students and OJT Coordinators on Documentation concerns.
CDAP UNDERTAKINGS FOR THE 2ND
SEM SCHOOL YEAR 2021-2022
•A. Distribution of class load to OJT coordinators
•B. Pre-deployment orientation of students and Coordinators
•C. Submission of the list of student interns to partner HTE’S
•D. Submission of Endorsement letter to host HTE
•E. Regular monitoring of student interns
•F. Distribution of the Performance Evaluation Monitoring form of Interns to host HTE
•G. Submission of the Performance Evaluation Monitoring form and Practicum report form
to the class OJT Coordinator.
•H. Submission of OJT grades for VPAA signing
•I. Submission of grades to the College Registrar
IMPORTANT
NOTES
▪ Practicum coordinators shall provide students a list of approved
departmental partner companies. Students may look for other
companies as long as the department, through the coordinator,
ensures that the company will provide appropriate training to our
students. Students are responsible for inquiring, applying, submitting
resumes, and informing companies about the templates for Moa's
between companies and DFCAMCLP, as well as between companies
and the student.

IMPORTANT NOTES

• Before sending internship plans to DFCAMCLP CDAP, the OJT coordinator will instruct
students to fill them out accurately and completely.

• The Moas must be submitted to DFCAMCLP CDAP at least three (3) weeks before the OJT
begins.

• For the initial approval of the MoAs, DFCAMCLP CDAP collaborates with the Office of the
VP for Academics.

• After notarization, DFCAM CDAP will forward/coordinate with VPAA for the final
approval of MoAs.

• CDAP will inform the OJT coordinator of the status of the MOA.
IMPORTANT
NOTES

There should be three (3) original signed copies of the MoAs.

The student is responsible for notarizing the Memoranda of Agreement between


the company and the student. The cost of the institutional MoA's notarization will
be covered by the students.

The internship won't begin until the MoAs have been signed (to ensure strict
compliance).

Only the agreement between the student and DFCAMCLP will be necessary in
situations when the student will complete an internal internship (work in a
division or office of DFCAMCLP).
OJT – AGREEMENTS: KEY
POINTS
1. Duration of the OJT shall not be more than five (5) months;
• EXCEPT for highly technical programs, which shall be in accordance with the
• Department’s existing policies, standards, and guidelines (PSG’s)
2. DFCAMCLP CDAP is to maintain a pool of qualified Host Training
Establishments (HTE)
a) Must be a juridical entity (partnership/corp) or government agency
b) HTE must duly registered with the respective regulatory body (SEC, LGU, DTI, etc.)
3. DFCAMCLP to provide appropriate insurance coverage for students
4. DFCAMCLP CDAP and HTE to assign their own internship coordinator
5. HTE to issue certificate of completion within two (2) weeks from end of OJT
6. Parent/Guardian of OJT Student to sign (expressing consent).
7. E-signatures allowed if coupled with a confirmation e-mail.
