PURPOSE:

ACTIVITIES AND THEIR CONTROLS

DEFINITIONS:
Hazards: Any condition/activity which has potential to cause the harm.
Risk: Outcomes from hazards. (Probability * Severity)
Control: Minimize/control the hazards/reduce the risk.

ACTIVITIES AND THEIR CONTROLS

1. Hazard: Electrical Shock, Electrocution, Arc

Associated Risks: Burn, Cut/Fracture, Fall, Death
Controls:
- Maintain the safe distance form electric equipment/cables/wire.
- Proper inspection/maintenance of electric equipments/tools.
- Use of adequate electric personal protective equipments .
- Do not touch the energized equipment until isolated.
- Wait until release of charge
2. Hazard: Noise
Associated Risks: Hearing loss/problem
Controls:
- Proper inspection/maintenance of equipments/tools.
- Use of earplug/muff in high noise areas. (MCW, HS Hall, Air
Compressor room, GT Comp)
- Shift the work to low noise area or move to low noise area intervally

3. Hazard: Slipping/Tripping
Associated Risks: Fall, Cut, Fracture
Controls:
- Always maintain the good housekeeping.
- Covering the trenches.
- Proper stacking/putting of tools/equipments/material at their proper place.
- Clearing of walkways/passages.
- Adequate lighting arrangement.

4. Hazard: Fire
Associated Risks: Burn, Death, Material loss
Controls:
- Proper inspection/maintenance of electric equipments/tools.
- keep away the combustible material from hot line of gas/steam/heat source.
- Hot work area should clear from combustible material before starting work.
- Follow the Hot Work Procedure.

5. Hazard: Rotating Part

Associated Risks: Cut, Injury, fracture
Controls:
- Do not touch the rotating part.
- Always remain away from rotating part.
- Cover the moving/rotating parts.
6. Hazard: Unsafe Chemical Handling
Associated Risks: Burn, Inhalation of chemical
Controls:
- Do not touch the chemical with bare hand.
- Use of adequate personal protective equipments .
- Read the label of chemical.
- Always follow the MSDS.
- Handle the chemical carefully.
- Cover the chemicals.

7. Hazard:Fall from Height

Associated Risks: Fracture, Death, Injury
Controls:
- Adequate plate form.
- Proper access or ladder or scaffolding
- Use of safety harness.
- Fastening of safety harness with firm support.
8. Hazard: Unsafe lifting with crane/hoist
Associated Risks: Material loss, Injury
Controls:
- Load should be under the lifting capacity.
- Crane/hoist should be inspected.
- Operator must has adequate knowledge about its operation.
- Never pass under the load.
- Barricading the area

9. Hazard: Ergonomics Hazard

Associated Risks: Muscles pain
Controls:
- Adequate sitting position/place.
- Follow the Ergonomics guidelines.
10. Hazard: Oil/chemical Spill
Associated Risks: Env Loss, Fire, Injury
Controls:
- Always use the spill prevention tray.
- Handle carefully.
- Clean / sweep the spilled oil/chemical immediately & properly.

11. Hazard: High Pressure/Hot gas/Steam line

Associated Risks: Burn, Inhalation of gas, Injury, Fire
Controls:
- Do not touch the High Pressure & Hot gas and steam line/equipment.
- Work on gas line after its purging with inert gas.
- Use of proper PPE’s.
- Dot not stay without work.
- Remain at safe distance
12. Hazard: Unsafe Manual Material Lifting
Associated Risks: Fracture, Injury
Controls:
- Lift the material safely.
- Do not over lift/over load.
- Position should be adequate.

13. Hazard: Confined Spaces Hazard

Associated Risks: Fire, Injury, breathing disturbance, Death
Controls:
- Follow the confined space entry procedure.
- Enter after the testing of gases.
- There should be adequate lighting arrangement.
- Adequate rescue arrangement
- Adequate communication
- Always enter two personnel.
- There should be one attendant at the entry points.
14. Hazard: Unsafe Welding/Grinding/Cutting
Associated Risks: Burn, Fire, Equipment damage, Property loss
Controls:
- Use of adequate personal protective equipments.
- Do not touch the energized equipment until isolated, drained & flushed.
- Keep away the combustible material from hot line of gas/steam/heat source.

Controls:
- Follow the Hot work procedure.
- Proper inspection/maintenance of equipments/tools.
15. Hazard: No Use of proper/adequate PPE,s
Associated Risks: Injury, burn
Controls:
- Always use adequate & proper PPE,s during plant visit & work.
16. Hazard: Heat Stroke/Heat Stress
Associated Risks: Headache, Unconscious
Controls:
- Drink plenty of cool water even if not thirsty
- Use the sun shade to prevent the direct sun heat
- Spend as little time as possible in direct sunlight
- In case of any symptom, contact to Clinic.
- Rotate work (from hot to cool areas)

17. Hazard: Poor Housekeeping

Associated Risks: Fire, Injury
Controls:
- Always maintain the good housekeeping.
18. Hazard: Unsafe Scaffolding
Associated Risks: Fall, Injury, Scaffolding Collapse, Death.
Controls:
- Follow the scaffolding safety procedure for scaffolding erection.
- Use adequate material (planks & pipes, joints).
- Proper Ladder.
- Sound base
- Adequate Pipe
- Hand rail
- Visual Inspection
- Use the Safety Harness.

CONTROLS:
 Check before the work.
 Follow the working and safety precaution mentioned in manual.
 Adopt the Safe Work Practices.
 Comply with PPE’s requirement and other safety rules.
 Maintain the housekeeping.
