RISK ANALYSIS

FOR LPG PLANT

PROBLEM STATEMENT
■ Degassing of the bullet was being performed in order to replace the magnetic level guage
by radar guages. The Plant had obtained all approvals for the replacement and hot permit
was issued.
■ For degassing of bullet no.3 fire hose was connected. To speed up the water filling for
degassing,another fire hose was connected from another double hydrant. For this
purpose,top manhole was opened. The person tried to hold the fire hose in manhole and in
this process one person fell inside the bullet and the second person went inside the bullet
to save first person.
■ Due to lack of oxygen inside the bullet and small size of manhole, no one could enter
inside the bullet and it leaded to the death of those 2 people.
PURPOSE OF THE STUDY
The importance of this case study is for us to understand:
■ What actually went wrong in the site.There was a Lack of supervision, and non -
compliance of work permit system.Emergency handling response- rescue operation could
not be done due to the non-availability of proper equipment for safe entry in vessel.
■ Lack of proper training to officers: the HSE officer had not undergone training and
management of change was not applied.
RECOMMENDATIONS
■ By following the starnding Operation Procedures known as the
■ SOP.
■ Degas the bullet by filling water inside from bottom liquid.
■ Check LEL and Oxygen level inside bullet.
■ Clean the bullet from inside.
■ Purge the bullet with LPG vapur drain water from the bullet.
■ Proper training has to be provided for all the workers.
■ Strict adherence to work permit system as per OISD-STD.

■ Generate threat zone estimates for various types of hazards. ALOHA can model toxic
gas,clouds, flammable gas clouds, BLEVES, jet fres, pool fires, and vapor cloud
explosions
■ By giving the latitude and longitude of the location and the time of the incident.
■ The threat zone estimates are shown on a grid in ALOHA, and they can also be plotted
on naps in MARPLOT. The red threat zone represents the worst hazard level, and the
orange and yellow threat zones represent areas of decreasing hazard.
