5. SCENARIO

(An employee A - 230230230), was working on scaffolding inside the Fire water storage tank at height. Hearing someone call from
outside the tank, he quickly unfastened the lanyard and descending through the ladder.
Due to lack of attention, he did not fully secure his foot on the rungs of the ladder and fell down. He tried to get up by himself but could
not. He was unable to call other workers, because of the trauma from the fall. Hearing the sound of falling, the hole watcher immediately
informed the Foreman (An employee B - 230250250) who was there about the incident then informed the ERT.
One of ERT member entered inside the tank and administered appropriate first aid to the injured person based on his condition and
immediately informed SICIM’s Emergency medical team. Accordingly, SICIM’s medical team and ambulance reached tank area.
During this time, the injured person was brought out from inside the tank by ERT and medical team through the tank shell man hole. He
was taken by ambulance to the nearest hospital for further medical care.
HSE officer (Hussain – 1011032323) notifies and reports the whole scenario to the environmental coordinator and the HSE manager.
