• Several tests are available to diagnose rabies in live patients, but no single
antemortem test is adequate
• An immunofluorescent staining technique
• To detect rabies antibodies in the cutaneous nerves at the base of the hair follicles

• A test that can detect the presence of viral nucleic acid (RNA)
• The blood tests came back positive for the presence of rabies
virus antigen

Part 2 – Treatment
• David’s doctor prescribes prophylactic treatment
• David is given
• a series of intramuscular injections of human rabies immunoglobulin
• a series of rabies vaccines
Part 3 – Saliva testing continued
• David’s saliva was also collected for viral cultivation
• In general, no single diagnostic test is sufficient for antemortem diagnosis
• The results will depend on the sensitivity of the assay
• The quantity of virions present at the time of testing
• The timing of the assay
• Release of virions in the saliva can vary
• The result was negative for viral cultivation from the saliva.
• One negative result is not an absolute indication of the absence of infection
• It may be that the number of virions in the saliva is low at the time of sampling
• It is not unusual to repeat the test at intervals to enhance the chance of detecting higher virus load
• Should David’s doctor modify his course of treatment based on these test results?

of treatment
• Given the positive RT-PCR test, it is best not to rule out a possible rabies
infection

• Near the site of the bite, David receives an injection of rabies immunoglobulin
• Attaches to and inactivates any rabies virus that may be present in his tissues
• Over the next 14 days, he receives a series of four rabies-specific vaccinations in the
arm.
• These vaccines activate David’s immune response and help his body recognize and fight the virus.
• Thankfully, with treatment, David symptoms improve and he makes a full recovery.
