Use of FMEA analysis

1. Identifying key process steps. A Process Plan or Process Flow Chart should be used.
If the process is complex, it should be divided into smaller units (SF, Lines, CNC).
2. Identifying failure modes: analyzing and thinking about the process and determining where failure might occur.
3. Analysis and definition of the root of the problem
4. Defining the consequences of problem and what effects it would have on the customer
5. Evaluating the probability, severity and possibility of failure detection

6. Calculation of the Risk Priority Number (RPN):
The RPN is obtained by multiplying the three factors (occurrence, severity, and detection) together.
The higher the number of RPNs, the higher the priority of that potential problem.
7. Defining the action plan: Based on the list of priorities, defining activities, responsibilities and deadlines.
8. Recalculating the RPN: After implementing the changes, repeating the FMEA to evaluate the effectiveness of the solution.

Model and database
- WNK uses a model in which it is possible
to select options from a drop-down menu.
- Each selection directs the team members
and based on it a new specific drop-down menu open
- When choosing the cause of failure, the model based on previous analyses
shows the effects of failure and recommends actions.
- The team can accept recommended actions or introduce new ones

- The model uses a database, which takes into account all previous FMEA analyzes
and solutions that the teams have come up with.
- After each new FMEA analysis, if a solution was found that was not applied before,
it is necessary to update the database
- By using FMEA analysis, many side effects are prevented before they happen,
and with continued use the base becomes stronger,
and solutions are easily and quickly applied to other products and processes.
