  CONSENT OF PARENT/ GUARDIAN

__________________________, a Grade 12 student of Aurora Senior High School, Aurora, Isabela, hereby permit/authorize him/her to undertake Work
Immersion as an academic requirement for graduation.
 
I agree that the said Work Immersion be conducted home based this School Year 2021 – 2022. I understand that the conduct of the Work Immersion will be
guided by the rules, regulations, policies and standard operating procedures of the school.
 
I hereby further agree to shoulder the personal and incidental expenses that may be incurred by him/her while undergoing the work immersion. I understand
that the school will not be held responsible for any untoward incident pertaining thereto.
