RUSSIAN CURRENT

INTRODUCTION
• Russian current also called a burst mode alternating current
• Medium frequency current
• Russian scientist Dr.Yakov M kots and co worker laid the foundation for the use
of Russian current in physiotherapy
DEFINITION

• Russian currents are alternating currents (AC) at a frequency of 2500Hz that are
burst, modulated at a frequency of 50Hz with a 50% duty cycle.
• Russian current have the ability to generate greater muscle force than maximal
voluntary muscle contraction and significant gain in muscle strength and
reported to be effective in the management of athletic type injuries.
CHARACTERISTICS

• Carrier frequency : 2500Hz

• Waveform : polyphasic sinusoidal waveform
• Current amplitude : 100mA
• Burst frequency : 50Hz
• Burst duration : 10ms
• On/off ratio : the on: off ratio is defined as the ratio of the tome during which there is
stimulation( contraction) to the time during which there is no stimulation ( no contraction). Set
as 10ms:10ms
• Duty cycle : duty cycle is the proportion of on time to the stimulation of both the on and off time
expressed as percentages. Russian current produced with duty cycle of 50%.
• Duration of treatment : the recommended duration of treatment is 10 min.
• Electrode placement : arranged parallel to the direction of muscle Fibers.
• Time delay switch :
 synchronous mode ( both channels triggered simultaneously)
reciprocal mode ( channel 1 on then 2 is off achieved by setting time delay equal
to time of channel 1)
 overlapping mode ( channel 2 overlaps with the time on and off time of channel

PHYSIOLOGICAL EFFECT

The net physiological effect depends on

• The total number of bursts delivered per second
• Russian current have the ability to depolarized
sensory nerve fibers - moderate prickling sensation
motor nerve fibers - tetanic muscle contraction
Depolarization of
sensory and
motor nerve
Medium Fibers, High level

motor units.
THERAPEUTIC EFFECT

To improve muscle strength To reduce muscle spasm To reduce oedema

Amplitude: tetanic muscle Amplitude: tetanic muscle Amplitude: small visible
contraction contraction contraction
Pulse rate(burst frequency) : Pulse rate: 50-70Hz Pulse rate : 35 – 50Hz
50 – 70 Hz Pulse duration : 50-175µs Pulse duration – 400µs
Pulse duration: 150- 200µs Stimulation applied to provide On time 1-2 sec off time 1-2
Apply current during volitional brief isometric contraction on sec promote muscle contraction
activities( voluntary activities) time
– isometric exercise at several On time 5-12, off time 8 -

INDICATIONS

• The only indication for use of Russian current as introduced by kots in 1977, is
to strengthen the muscular system of healthy and athletic person.
• Post knee ligaments surgery
• Post arthroscopic knee surgery
• Following ligament sprain
• Muscle spasm
CONTRAINDICATION

• Cardiac pacemakers and arrhythmias

• Haemorrhagic conditions
• Thrombosis and thrombophlebitis
• Unconscious patient
• Malignancy
REBOX CURRENT

• Rebox electrotherapeutic method is based on non invasive transcutaneous application of specific

electric current to a living tissue.
• The rebox device was developed in Czechoslovakia in 1970’s. the rebox current consist of
unipolar rectangular pulses of between 50µsec and 250µs at 300Hz. Thus it is a medium
frequency current.
• The rebox is a unique development in the filed of electrotherapy for the treatment of pain in
abroad range of soft tissue injury.
• The name rebox derived from the “rehabilitation box”
• MECHINE CIRCUIT
• There is a hand held mechine and current is delivered by a point type electrode.
• The circuit is also contain micro ammeter and earphone connected to a small computer monitor
to display a graph of current and other parameter.
• Rebox directly provide feedback from tissue during treatment, thus enabling the therapist to
work more effectively.
• It may allow the therapist not only to monitor objectively a patient’s progress but also to vary the
treatment dosage in accordance to the degree of injury.
PARAMETERS
• Waveform : rectangular pulsed mono phasic waveform
• Frequency : 300Hz
• Pulse duration : 100µs
• Current amplitude : 0 -300µAmp
• APPLICATION
• When the rebox current is applied to normal tissue it rises over a period of about 1 sec
characteristic displacement of the meter, of sound is produced in the earphone and shape of the
graph.
• When the rebox current is applied to damage tissue/abnormal tissue, the rise of current is slow,
taking 3 to 4 sec may occur series of steps.
• For the therapeutic benefit apply the rebox current repeatedly for a few seconds over the
damaging area.
PHYSIOLOGICAL EFFECT

of osmotic
condition
THERAPEUTIC EFFECT

• A rapid increase in capillary blood flow

• Reduction of oedema
• Increase in lymphatic drainage
• Reduction of muscle tension
USES
• Used for symptomatic relief of chronic pain
• Used for traumatic pre and post operative pain relief
INDICATIONS

• Rebox has ben showing beneficial for the treatment of pain in soft tissue injury.
• Frozen shoulder
• Lateral epicondylitis
• Oedema
• Over use syndromes
• Post operative trauma
• Rheumatoid arthritis
CONTRAINDICATION

• Neoplasm
• Bacterial infection
• Pregnancy or delivery
• DVT
• The carotid sinus region
• The principal difference between rebox and other form of electrotherapy are:
• Very low energy output
• Unique feedback method for determination of dosage
• Brief duration of application
