The team rendered their exit call with PLTCOL ALEX DJ ALBERTO,
Station Commander and at the same day with Chief, SIDMB, PMAJ
BENJAMIN D MAYOR JR
OBSERVATION
 We observed that nobody is checking the temperature of all personnel/visitors entering the
Station, likewise equipment (thermometer) is not usable.
 We observed that the station does not have proper filing of Incident Record Form same with
CIRAS.
 Custodial Facility is loaded more than its capacity.
 A comfort room was used as Female Custodial Facility.
 The entry and data in the blotter book were updated and complete
 Street Dogs were inside the lobby area of the station.
 SWCPD is headed by a PNCO with the rank of PEMS.
RECOMMENDATION
 All personnel/visitors should be assessed before entering the premises Station 10 for
symptoms of COVID-19 and ensure that equipment are usable.
 Proper filling of documents should be observed at all times.
 The population of the PUPC should be addressed by the station commander to
prevent sickness and infections especially Covid-19 and request for additional facility
to accommodate the volume of PUPC.
 Give consideration to the Female PUPC for their health and safety.
 It is recommended that the station sustain their complete and updated entry on the
blotter book and consider as one of their best practices
 Street Dogs inside the lobby area of the station should be addressed by the station.
 The WCPD should have its own chief, preferably PCO for handling Gender Based and
minor related incidents.

PLACE OF INCIDENT Blk 40, Brgy Addition Hillsm Mandaluyong City.

On the said date, time and place suspect was caught in the act selling one (1) heat-sealed transparent plastic
sachet allegedly containing Php500.00 worth of Shabu to a poseur buyer. The marked money was recovered
from the suspect and another two pcs of heat-sealed transparent plastic sachets also containing what is
FACTS OF THE CASE alleged to be shabu. The conduct of markings and inventory of seized items was done at the place of
operation and in the presence of the suspect, Brgy. Councilor Danilo Salvador, and a media representative. A
laboratory examination was conducted on the three pieces of heat-sealed transparent plastic sachets
containing the alleged shabu, and yielded positive results for Methamphetamine Hydrochloride, a prohibited
and dangerous drug.

On the said DTPO, victim and the two suspects were having a verbal altercation inside the victim's room and
not long after, successive gunshots were heard by the witnesses. The suspects fled away and the household

City Medical Center but was pronounced dead on arrival by the attending physician. Ronel Cater Hondrada
later attested to witnessing the incident and identified the suspects.
