DEFINITION
ENVIRONMENTAL MANAGEMENT SYSTEM (ISO 14001)
• Environmental Aspect : Element of an organization’s activities or products or services that
can interact with the environment
• Environmental Impact : any change to the environment, whether adverse or beneficial,
wholly or partially resulting from an organization’s environmental aspects
• Significant Environmental Aspect : A significant environmental aspect has or can have a
significant environmental impact.

OCCUPATIONAL HEALTH & SAFETY ASSESSMENT SERIES (OHSAS 18001)


• Hazard : Source, situation, or act with a potential for harm in terms of human injury or ill
health, or a combination of these
• Risk : Combination of the likelihood of an occurrence of a hazardous event or exposure(s)
and the severity of injury or ill-health that can be caused by the event or exposure(s)
• Risk Assessment : Process of evaluating the risk(s) arising from a hazard(s), taking into
account the adequacy of any existing controls, and deciding whether or not the risk(s) is
acceptable.

DEFINITION
Abnormal : Deviation from normal condition and/or operation condition such as black out, the
capacity is not conform, human error, new or untrained employment, equipment
break down/not functioning/maintenance, chaotic disturbance, natural disaster (that
create industry accident), or deviation standard procedure.

Acceptable Risk : Risk that has been reduced to a level that can be tolerated by the organization having
regard to its legal obligations and its own OH&S Policy

Accident : An incident which has given rise to injury, ill health or fatality

AMDAL : Analisis Mengenai Dampak Lingkungan (Analysis of Environmental Impact) The


process of studying the significant impact of a proposed business or activity on the
environment, which is required as part of the decision-making process =
Environment Impact Assessment (EIA)

Classification of Environmental : The classification of each environmental aspect and impact using the significant
Aspect and Impact aspect criteria to determine whether it is significant or not significant

Cost Effect : The financial impact either capital, operational, variable expenses caused to PT.

Emergency : Situation or condition that can be lead to accident, fire, spill/leakage as a result from system failure.

Environmental Aspect : Element of an organization’s activities or products or services that can interact with the environment.

Environmental Impact : a. (UU no. 32 of 2009) : a change in the environment caused by an activity.
b. (ISO 14001) : any change to the environment, whether adverse or beneficial, wholly or
partially resulting from an organization’s environmental aspects

First Aid Case (FAC) : A work related minor injury or illness which can be treated by a first aider or equivalent and does not
require a professional physician or paramedic

Hazard : Source, situation, or act with a potential for harm in terms of human injury or ill health, or a
combination of these.

Ill Health : Identifiable, adverse physical or mental condition arising from and/or made worse by a work activity
and/or work-related situation

Incident : Work-related event(s) in which an injury or ill-health (regardless of severity) or fatality occurred, or
could have occurred.

Medical Treatment Case (MTC) : The injured or sick person requires treatment (more than first aid) from a professional physician or
qualified paramedic

Near-miss/near-hit/close call/dangerous : An incident where no injury, ill health, or fatality occurs

Non routine : All activities that is conducted on certain period/duration and does not include as main core activities, such as
welding repair, etc.

Restricted Work Case (RWC) : Where the employee cannot fulfill his normal work the day following an incident but is able to undertake
a temporary job; work at his normal job but not full-time; work at a permanently assigned job but
unable to perform all duties normally assigned to it

Risk : Combination of the likelihood of an occurrence of a hazardous event or exposure(s) and the severity of
injury or ill-health that can be caused by the event or exposure(s)

Risk Assessment : Process of evaluating the risk(s) arising from a hazard(s), taking into account the adequacy of any
existing controls, and deciding whether or not the risk(s) is acceptable.

Significant Aspect Criteria : The criteria established in this procedure to evaluate and determine the magnitude of significance for
each environmental aspect.

Significant Environmental Aspect : A significant environmental aspect has or can have a significant environmental impact.

Significant Environmental Impact : A significant environmental aspect that has or can have a significant environmental impact shall be
followed up with objectives, targets and programs

Workplace : Any physical location in which work related activities are performed under the control of the
organization

The environmental aspect and impact and/or occupational hazard & risk from
PT. YAMA Engineering consist of several item from :
• Listing the environmental aspect and impact and/or the occupational hazard
& risk of every PT. YAMA Engineering material, activity , working methods,
tool, environment, product & service.
• The actual or estimated capacity or amount of any material, activity, tool,
workplace, product & service handled, should be identified as possible
• Classify the status of significance for each aspect/hazard and impact/risk
referring to the established criteria given in this procedure

PARAMETER OF HAZARD IDENTIFICATION OF RISK AND
ENVIRONMENTAL IMPACT

Likelihood
• The likelihood that the potential causes of the aspects/hazard will occur when the
activity/ process is being under taken.
• The likelihood that hazard/ aspect will occur/expose when the activity/ process is
being under taken.
• The likelihood that the incident occur either for health and safety or environmental

Severity
• The consequence of aspect or hazard to the environment,
• health and safety of the personnel and other properties.
• Severity is an assessment of the serious of the impact on the environmental or risk on
health and safety.
• Severity applies to the effect of risk/impact.
• Level of Severity can be decreased if there are preventive actions that has been done
to reduce the seriousness of the hazard/ aspect

LIKELIHOOD PARAMETER
Likelihood Description
5 - Daily to monthly event (larger exposure durations).
Frequent - The potential hazard/aspect occurs in daily event during working hours.
- The potential hazard/aspect occurs for non routine activities with long working duration that exist for at least 8 hours or
accumulative one week working duration.
- The frequency of incident happen is more than once in six month.
- Current controls cannot/ fail to detect and/or control the hazard/ environmental aspect or impact from hazard/ impact
for environmental and have high possibility to happen
4 - Monthly to semester event (exposure remaining limited in duration)
Probable - The potential hazard occurs more than once in a month but with short duration.
- Abnormal condition. The potential hazard occurs due to human error or equipment failure.
- Non routine activities, the potential hazard/ aspect occurs more than once in a month with duration more than one hour.
- The potential hazard associate when personnel working in hazardous area or exposed by hazard in hazardous area but
not constantly happen.
- The frequency of incident happen is more than once in six month.
- Current controls will detect and/or control the hazard/ environmental aspect or impact from hazard/ impact for
environmental but still often to happen.

3 - Monthly to annual event (exposure remaining limited in duration)


Rare - The potential hazard occurs more than once in a month but with short duration.
- Non routine activities, the potential hazard/ aspect occurs more than once in a year with duration more than one hour.
- The frequency of incident happen is once in six month.
- Current controls will detect and/or control the hazard/ environmental aspect or impact from hazard/ impact for
environmental but still possible to happen.

2 - Events occurring every few years (short and rare exposure).


Remote - The potential hazard for non routine activities occurs in unavoidable condition, emergency condition and force major.
- Frequency of incident happen is once in a year.
- Current controls will detect and/or control the hazard/ environmental aspect or impact from hazard/ impact for
environmental so it will be never happen/difficult to happen or rarely happen.

1 - Events occurring very rarely.


Improbable - No potential hazard or aspect from activities.
- Current control is suitable to prevent incident.
- No health, safety and environment incident.

SEVERITY PARAMETER (FOR HUMAN)

5 - Permanent disability, mass hospitalization, death, large scale permanent environmental impact/damage,

Catasthropic - Current control is not applicable to reduce the severity of the hazard/ impact.

4 - Amputation; major fractures; positioning multiple fractures; fatal injury, occupational cancer; other illnesses
High reducing life expectancy. Partial permanent incapacity to work.
- If the impact causes permanent environmental damage, fatality to human, permanent/partial disability, total
destruction that cause total property damage , interruption in working progress more than one week and/or
causes loss day more than one week.
- Current control is not applicable to reduce the severity of the hazard/ impact.
3 - Lacerations and wounds; burns; concussion; serious sprains; minor fractures; deafness; dermatitis; asthma.
Medium Poor health leading to a minor but permanent disability; or temporary incapacity to work.
- If the environmental impact is limited, the recoverable within 1 month to 1 year
- If a major injury occurs and needs hospitalization and/or causes loss day in 2 days to 1 week.
- If the impact causes temporary disability, interruption in working progress in one working shift to one week,
property damage that recovery in one week.
- Current control is applicable and able to reduce the level of severity from high level to medium level.

2 - - Incidents nearly without injury or with superficial injuries; small cuts and bruises; irritation of eyes by dust;
Minor discomfort or problem with temporary discomfort of one or several people. Benign events or accidents that
are recorded and/ or cause one day loss, or :
 If the result of the impact is minimum and immediately recoverable in one day.
 If the impact causes interruption in production process in one hour to half of working shift, property
damage that recovery in one day.
- Current control is applicable and able to reduce the level of severity from Medium to Minor.
1 - If there is no impact or the impact is very small/minor to human, property, production process,
Negligible environmental or if an injury occurs but can be sufficiently treated by the First-Aid team/ physician's

Risk level Control of the risk to be considered

Work cannot be continued without reducing the risk. If reduction is


impossible, even using very major resources, this work must remain

ROLE AND RESPONSIBILITY
Person In Charge (PIC) :Representative of related department has responsibility to
prepare and conduct Hazard Identification/Environmental
Aspect, Risk/Impact Assessment & Risk/Impact Control (HAZID)
for their activity
HSE Department : responsibility to review and give advice to the department
representative according to preparation to conduct HAZID
Supervisor/Engineer : has responsibility to review and give approval for HAZID before
HAZID can be implemented on Site.
Head of Department : has responsibility to review and give approval for HAZID before
HAZID can be implemented at Office.
Document Control : Distibute the controlled copy of Hazard & Aspect Identification,
Risk & Impact Assessment and Control to HSE-MR/Project HSE
Committee, HSE Officer/Engineer/HSE Site On Duty,
Project/Department Manager & all relevant parties as specified
by Dept./Project/Construction Manager. To add access places for
controlled copy of Hazard & Aspect Identification, Risk & Impact
Assessment and Control shall be processed using change
proposal.

HAZARD & ASPECT IDENTIFICATION, RISK & IMPACT
ASSESSMENT AND CONTROL REVIEW

Review of Hazard & Aspect Identification, Risk & Impact Assessment and Control shall be
made:
• at least once every a year
• if there is any change in operation or scope of
material/activity/tool/workplace/product/service.
• if there is any change in legal or other requirements that affected particular
hazard/risk or aspect/impact.
• When review of Hazard & Aspect Identification, Risk & Impact Assessment and
Control shows there is change in operation/lay out of material, activity, tool or
product and classified significant. The Construction/Project/Department Manager
and/or HSE-MR and/or Project HSE Committee will study/evaluate whether it
becomes potential source of emergency in accordance with YM2–HSE–MR07 (Criteria
for Determining Potential Emergency) and it should be monitored or measured
regularly in accordance with YM2–HSE–MR08.
