Occurence
Acceptable Risk : Risk that has been reduced to a level that can be tolerated by the organization having
regard to its legal obligations and its own OH&S Policy
Accident : An incident which has given rise to injury, ill health or fatality
Classification of Environmental : The classification of each environmental aspect and impact using the significant
Aspect and Impact aspect criteria to determine whether it is significant or not significant

Environmental Aspect : Element of an organization’s activities or products or services that can interact with the environment.

partially resulting from an organization’s environmental aspects
First Aid Case (FAC) : A work related minor injury or illness which can be treated by a first aider or equivalent and does not
require a professional physician or paramedic
Hazard : Source, situation, or act with a potential for harm in terms of human injury or ill health, or a
combination of these.
Ill Health : Identifiable, adverse physical or mental condition arising from and/or made worse by a work activity
and/or work-related situation
Incident : Work-related event(s) in which an injury or ill-health (regardless of severity) or fatality occurred, or
could have occurred.
Medical Treatment Case (MTC) : The injured or sick person requires treatment (more than first aid) from a professional physician or
qualified paramedic
Restricted Work Case (RWC) : Where the employee cannot fulfill his normal work the day following an incident but is able to undertake
a temporary job; work at his normal job but not full-time; work at a permanently assigned job but
unable to perform all duties normally assigned to it
Risk : Combination of the likelihood of an occurrence of a hazardous event or exposure(s) and the severity of
injury or ill-health that can be caused by the event or exposure(s)
Risk Assessment : Process of evaluating the risk(s) arising from a hazard(s), taking into account the adequacy of any
existing controls, and deciding whether or not the risk(s) is acceptable.
Significant Aspect Criteria : The criteria established in this procedure to evaluate and determine the magnitude of significance for
each environmental aspect.
Significant Environmental Aspect : A significant environmental aspect has or can have a significant environmental impact.
Significant Environmental Impact : A significant environmental aspect that has or can have a significant environmental impact shall be
followed up with objectives, targets and programs
Workplace : Any physical location in which work related activities are performed under the control of the
organization

Likelihood
• The likelihood that the potential causes of the aspects/hazard will occur when the
activity/ process is being under taken.
• The likelihood that hazard/ aspect will occur/expose when the activity/ process is
being under taken.
• The likelihood that the incident occur either for health and safety or environmental
Severity
• The consequence of aspect or hazard to the environment,
• health and safety of the personnel and other properties.
• Severity is an assessment of the serious of the impact on the environmental or risk on
health and safety.
• Severity applies to the effect of risk/impact.
• Level of Severity can be decreased if there are preventive actions that has been done
to reduce the seriousness of the hazard/ aspect
4 - Amputation; major fractures; positioning multiple fractures; fatal injury, occupational cancer; other illnesses
High reducing life expectancy. Partial permanent incapacity to work.
- If the impact causes permanent environmental damage, fatality to human, permanent/partial disability, total
destruction that cause total property damage , interruption in working progress more than one week and/or
causes loss day more than one week.
- Current control is not applicable to reduce the severity of the hazard/ impact.
3 - Lacerations and wounds; burns; concussion; serious sprains; minor fractures; deafness; dermatitis; asthma.
Medium Poor health leading to a minor but permanent disability; or temporary incapacity to work.
- If the environmental impact is limited, the recoverable within 1 month to 1 year
- If a major injury occurs and needs hospitalization and/or causes loss day in 2 days to 1 week.
- If the impact causes temporary disability, interruption in working progress in one working shift to one week,
property damage that recovery in one week.
- Current control is applicable and able to reduce the level of severity from high level to medium level.
2 - - Incidents nearly without injury or with superficial injuries; small cuts and bruises; irritation of eyes by dust;
Minor discomfort or problem with temporary discomfort of one or several people. Benign events or accidents that
are recorded and/ or cause one day loss, or :
If the result of the impact is minimum and immediately recoverable in one day.
If the impact causes interruption in production process in one hour to half of working shift, property
damage that recovery in one day.
- Current control is applicable and able to reduce the level of severity from Medium to Minor.
1 - If there is no impact or the impact is very small/minor to human, property, production process,

12. Risk Ranking : Level of Risk according to result from risk matrix;
Tolerable, Moderate, Substantial, Intolerable.
13. Proposed Risk Control : Risk control that is proposed to reduce the level of
risk into tolerable risk. That is including a preventive
action and legal/procedure/standards compliance.
Review of Hazard & Aspect Identification, Risk & Impact Assessment and Control shall be
made:
• at least once every a year
• if there is any change in operation or scope of
material/activity/tool/workplace/product/service.
• if there is any change in legal or other requirements that affected particular
hazard/risk or aspect/impact.
• When review of Hazard & Aspect Identification, Risk & Impact Assessment and
Control shows there is change in operation/lay out of material, activity, tool or
product and classified significant. The Construction/Project/Department Manager
and/or HSE-MR and/or Project HSE Committee will study/evaluate whether it
becomes potential source of emergency in accordance with YM2–HSE–MR07 (Criteria
for Determining Potential Emergency) and it should be monitored or measured
regularly in accordance with YM2–HSE–MR08.
