CASE STUDY 1: SUBSTANCE ABUSER
 Name : FZ
 Age :24 years

 Sex : male

 Occupation : student

 Address : nabha

 Chief complaints: Acute substance abuse, felling depressed, general fatique,worthlessness, suicidal thoughts,
nausea, pain in head and back
 Informant : brother

 History of illness: 1.duration : 8 years


2. physical factor : easily availability of alcohol
3. Psychological factor : no emotional contact , strict father, rebellious attitude
4. onset: At the age of 16 years
5.psychiatric history : on antidepressants for past 2 years
 Mental status examination :

--Consciousness: alert –general appearance : well groomed –memory : good –speech: rate low, pitch and tone low
 Test administered : FAHMA(Functional assessment of mental health and addiction scalr)

 RESULT : FAHMA Score : 160.44

 Provisional diagnosis : SUMI ( SUBSTANCE USER MENTALLY ILL)

 SUGGESTED INTERVENTION :

1. Meditation
2. Cognitive therapy
CASE STUDY 2: DRUG ADDICTION
 Name : Ms K
 Age :29 years

 Sex : female

 Occupation : IT Software Engineer

 Chief complaints: Resigned job, can’t carry routine work,

 Informant : brother

 History :

Psychological factor : mother died in early age , not so healthy relation with father,
worried about brother future , stressed, grief
 Mental status examination :

- Consiousness: alert –general apperance : expression of mortification, beautifully


dressed up –memory : good –speech: low, fumbling, brittle voice
 Test administered : FAHMA

 RESULT : FAHMA Score : 155.32

 Provisional diagnosis : Drug addict

CASE STUDY 3 : FAMILY COUNSELING

 Chief complaints: Doesn’t like anyone interfering in life and blames mother for
issue in life
 Informant : Mother

 History :

Psychological factor : not good relation with mother,


 Mental status examination :

- Consiousness: alert, notorious –general apperance : not proper eye contact well
groomed –memory : good –speech: rate low, pitch and tone low
 Assessment method : session interview

 Provisional diagnosis : Dysfunctional parents, emotional and behaviour problem

 SUGGESTED INTERVENTION :

1. Strategic Family therapy of 12-15 sessions


CASE STUDY 4: MULTIPLE SUBSTANCE
ABUSE
 Name : RZ
 Age : 17 years

 Sex : male

 Occupation : property dealer

 Chief complaints: loss of appetite,getting unconscious,head ache,vomiting, aggressive, disobedient, fever,


depression, body pain, urge to smoke, consume weed/hash/cocaine/alcohol and cough syrup, bunk classes
 Informant : Parents

 History : : 1.duration : 5 years


2 . Psychological factor : peer pressure
3.Medical history : admitted to hospital for 2 weeks
 Mental status examination :

- Consiousness: alert, energetic –general apperance : Appropriate –memory : intact –speech: Normal
 Assessment method : Digital span test, days test , month test , standard psychological test- pgi hq and BDI

 Inference : attention and concentration – aroused but difficulty sustaining

 Results : BDI score – 18 & PGI HQ score-59.9% in physical distress and 56.25% in psychological distress

 Provisional diagnosis : Moderate distress and boardline clinical severe depression

CASE STUDY 5: BIPOLAR DEPRESSION

 Age : 62 years

 Sex : female

 Occupation : govt employee

 Chief complaints: unconsious for few minutes, low bp, bed wetting,feeling sad,helpless
frustration,irritation and depression , suicidal thoughts, unnecessary interference in family ,body ache,rude
hyper-aggressive and loss of interest in daily work
 Informant : Husband

 History : : 1.duration : since after marriage


2 . Psychological factor : adjustment problem with inlaws after marriage,death of
brother and father
3.Medical history : depression for 2 years, heart problem,diabetic ,high bp and
thyroid problem, liver problem
 Mental status examination :

- Consiousness: alert, energetic –general apperance : Appropriate –memory : immediate memory slightly
impaired –speech: Normal
 Assessment method : Digital span test, days test , month test , standard psychological test- pgi hq and BDI

 Inference : attention and concentration – aroused but difficulty sustaining

 Results : BDI score – 37 & PGI HQ score-25% in physical distress and 72.72% in psychological distress

 Provisional diagnosis : Severe distress and severe depression

CASE STUDY 6: INTERNET ADDICTION

 Occupation : student

 Chief complaints: use internet for long hours and become habitual that led to
migraine
 Informant : Parents

 History of illness: 1.duration : 1 year


2. Psychological factor: spend 8-9 hours on internet
 Mental status examination :

- Consiousness: alert –general apperance : well groomed –memory : good –speech:


rate low, pitch and tone low
 Test administered : psychological interview

 Inference: spend more than 4 hours is addiction

 Provisional diagnosis : Addiction

CASE STUDY 7 : OPIAM ADDICTION

 Age : 32 years

 Sex : male

 Occupation : property dealer

 Chief complaints: severe head ache,vomiting,body pain, sneezing , yawning ,sad aggressive,
mood swings, diarrhoea and boredom in life
 Informant : Parental uncle

 History : : 1.duration : 10 years


2 . Psychological factor : to relive the body pain after labour he used
opium
3.Medical history : admitted to hospital once but no improvement
 Mental status examination :

- Consiousness: alert, –general apperance : Appropriate –memory : immediate memory

 Inference : attention and concentration – aroused but difficulty sustaining

 Results : BDI score – 37 & PGI HQ score-87.5 % in physical distress and 86.36 % in
psychological distress
 Provisional diagnosis : Severe distress and severe depression

CASE STUDY 8 : MOOD SWINGS

 Occupation : Housewife

 Chief complaints: loss of appetite, frequent headache, boredom,mood swings,short tempered , tired,
feeling worthless,insulted, ignored, and restless
 Informant : Close friend

 History : : 1.duration : since after 2 years of arrange marriage


2 . Psychological factor : Discriminating behaviour of mother in law

 Mental status examination :


- Consiousness: alert, energetic –general apperance : Appropriate –memory : intact –speech: Normal
 Assessment method : Digital span test, days test , month test , standard psychological test- pgi hq and
BDI
 Inference : attention and concentration – aroused and sustained

 Results : BDI score – 17 & PGI HQ score-62.5% in physical distress and 79.9% in psychological
distress
 Provisional diagnosis : Moderate distress and borderline clinical severe depression

 SUGGESTED INTERVENTION :

1. Rational Emotional Behaviour therapy


CASE STUDY 9 : HEROIN ADDICTION

 Occupation : Businessman

 Chief complaints: severe head ache,vomiting,body pain, shaking legs ,sad, aggressive, insulted,
guilt, frustrated
 Informant : Father

 History : : 1.duration : 2 years

2 . Psychological factor : lack of understanding/love/support from family ,distrust

3.Medical history : admitted to hospital once but no improvement


 Mental status examination :

- Consciousness: alert, –general appearance : Appropriate –memory : intact –speech:

and BDI
 Inference : attention and concentration – aroused and sustain

 Results : BDI score – 18 & PGI HQ score-43.7 % in physical distress and 77.20 % in psychological
distress
 Provisional diagnosis : Severe distress and mild depression

- Consciousness: alert, –general appearance : Appropriate –memory : impaired –speech: tone


and speech slow
 Assessment method : Mini Mental state Examination

 Inference : forgetful

 Provisional diagnosis : Dementia

2. Family therapy
3. MICBT
4. Psycho education
