Incident description:
On 12th March'23 at around 9:30 am at SMP-2, Mr. Sikandar Ray, a Sr Technician who has been working
almost 20 years with the organization was busy with regular shift activities. While he was doing regular job,
he has found a non-standard broom owned by some other co-contractor was kept in middle of the
temporary material storage area. After seeing this, he was wanted to shift the same to its designated place.
During shifting, the extended part of the broom got stuck in a storage box and his right feet got entangled

subsequently, resulted trip and fall in the same level. While falling, his leg was twisted due higher body
weight and he received injury on the ankle region. Swelling observed and he had been taken to the nearby
medical facility immediately for first aid and necessary treatment.

Initial findings:

• Necessary communication had been initiated in the shift with immediate stoppage of the activity.

• 8D PPS initiated to find out the root cause.

Incident date: 15th March’ 2023 at 06:45 PM
Safety Alert – DO (Not related to Vesuvius scope of work) Type of incident : Dangerous Occurrence
Place: SAIL RSP, SMS#2

Incident description:
On 15th March’23 at around 06:00PM, heat tapped in Ladle#10 (SGM maintained by Ladle#10
IFGL and Brick lining is done by RHI-Magnesita) from Converter#1. After tapping 150 Puncture Zone
Ladle#10 Shell Punctured from Metal zone area - DO

MT of liquid steel, ladle was being transferred to LHF through Overhead crane. While
transferring, at about 06:45PM, liquid metal started leaking from Metal zone area
rupturing Bottom ladle Shell. Due to the Spillage of Liquid Metal on the transfer bay, Metal Zone
track got jammed and the power cable got damaged due to spillage. Neither any injury
nor any major damage to property reported by customer.
The scope of mitigation is with customer. However, we’ve raised this issue in Line PPS
for proper communication and awareness among our people. RPN 2X16=32

Initial Findings:
 Bottom Ladle shell punctured..
 Life of the ladle was 40 Heats.
 No injury reported.

Immediate Actions Initiated:


 Incident will be shared with all employees in TBT.

Safety Alert – DO – Arc Furnace Shell punctured at Slag Zone Area (Not VIL work related) Place: CSP / SMS#1
Plant: BPSL-J

Incident description
On 11th Mar 2023 at around 09:10 AM, Arc Furnace Shell No. 2 got
punctured at slag zone area. Approximately 10 MT metal fell on ground &
on LT car, which lead to LT car cable burn and LTC movement was blocked

due to metal jam. It was noted that furnace life after repair was 267 and total
life was 580 heats. Customer foreman observed this metal leakage,
immediately stopped all activities until furnace is repaired. There were no
manpower at this location so no any injury reported.
RPN (LXS) : 2 X 16 = 32

Initial Findings:
• Arc Furnace shell punctured from Slag zone area
• Furnace Life after repair – 267.
• No injury reported.
Immediate Actions Initiated:
• Immediately all the works were stopped.
• Required repairs were carried out.
