1. PREFATORY STATEMENT

• Background and rationale on the mandatory audit and the shift to an


online-based platform

2. PURPOSE

• Endorse Supplemental Guidelines on 2021 CFLGA using the SCFLG KMS;


• Serve as reference guide for all implementing national and sub-national
agencies and structures; and
• Ensure effective and efficient implementation of CFLGA while adhering to
new normal protocols and minimum health standards.

review, and validation

4.2.4. User Registration


a. Individual User Profile and Account Registration Form (applicable to all
levels of IMTF):
• Accomplish the form and sign the Data Privacy Consent.
• Scanned copies must be submitted to the LGOO assigned to the IMTF
for consolidation.
b. Consolidated User Profile and Account Registration Form (applicable to
all levels of IMTF):
• Encode details from the Individual User Profile and Account
Registration Form.
• Submit excel file to LGOO assigned to the IMTF for consolidation.
4.2. Methodology

4.2.4. User Registration


a. All City, Municipal, and Provincial IMTF Chairs shall act on the following:
• Follow up and collate all Individual and Consolidated Registration
Forms of each IMTF member;
• Ensure all entries of both forms are consistent and correct; and
• Submit all scanned copies to the higher level IMTF.
b. DILG Regional Focal Persons and RC/SCWC Regional Coordinator shall
consolidate, review and endorse all forms via electronic mail to
ictsupport@cwc.gov.ph; scflg-kms@cwc.gov.ph; and
nboocflga@gmail.com with the subject: “SCFLG KMS: Region # IMTF
Users for 2021 CFLGA” (example: SCFLG KMS: Region V IMTF users
for 2021 CFLGA)

• The audit proper will start by June with the following pre, during, and post-

• RIMTF will submit Audit and Validation Summary Results (Forms 2 to 3)
