Agreement 1
Regarding PCW Certificates
● RHUs to write the training section of DOH – CAR thru Ms. Julie Morareng for the
updating of the status of PCW certificates.
● DMOs to follow up the training section regarding the status of PCW certificates not yet
given to RHU personnel in their assigned area.
Agreement 2
Medical Records Management Training
● RLED to give extension for the submission for Medical Records Management Training
Certificate until December 31, 2023 based on RLED advisory 2023-016.
● RLED recommends the RHU to craft a supplemental plan for the funding for Medical
Records Management Training of Records Officer.
● PHO representatives to refer to PHOfficer regarding this matter, esp. Because the PHO
has a training fund, and to give feedback to RLED as soon as it is done.
Agreement 3
ICD-10 Training Certificate
● RLED to give extension for the submission for ICD-10 Training Certificate
until December 31, 2023 based on RLED advisory 2023-016.
● DMOs to coordinate with LHSD on the presence of the names of Records Officers in
their assigned areas in the list of participants of ICD-10 to be given by the LHSD

Validated Electronic Medical Records
● For PDOHO Apayao (DMOs) to follow up DOH-CAR IT section regarding the
provision of CYPHER KEY for Iclinicsys to RHUs.

Filing
● For RHUs to sustain filing and maintain its presence at all times.

Logbooks
● RHUs to make sure that all records are updated.
● RLED to share a matrix/template for the logbooks
● DMOs to oversee if the templates provided by the RLED to the RHU have been used
and also to oversee whether all records have been updated.
Agreement 7
Contract with Infectious Waste Hauler

● RLED to give extension for the submission for Contract with Infectious Waste Hauler
until December 31, 2023 based on RLED advisory 2023-016.
● RLED to follow up DENR-EMB regarding the status and fast tracking of the issuance of
Waste Generators ID of the RHU.
Agreement 8
Revenue Code

● The RHU shall give a copy of the revenue code to RLED.

● Assigned LO shall coordinate with the facility.

Agreement 10
Correction of all deficiencies and revisiting of RLED
● The RHU committed to correct all deficiencies by next quarter (3 rd Quarter) (July to
September)
● The PHO may schedule activities on the last week of July (But still for approval of the
Provincial Health Officer)
● The PDOHO shall spearhead the crafting of an action plan indicating the timelines and
activities to be done to correct the deficiencies of Calanasan RHU and the visiting of
Calansan RHU by the RLED for the validation of corrective measures done.
● RLED recommends that all plans be submitted before August 2023, this is before the
scheduled external audit to be done by other CHDs on CHD CAR.
