A concrete mixer truck driver on his first trip to a construction site overturned his vehicle after going off the road at 41 km/h in a 60 km/h zone. The incident was likely due to dark conditions, overconfidence as a new driver, and distraction from a phone call. The company learned that self-awareness of mental states, frequent risk analysis, and avoiding distractions like phones are important for safety. Going forward, the company will promote safety trainings, risk analysis apps, anti-distraction messaging, and ensure lessons are understood to prevent similar incidents.

What? • On July 8, 2021 at 05:47 a.m., the driver of a concrete mixer truck, on his first trip to the construction site, was driving at a speed of 41 km/h on a road limited to 60 km/h max. For reasons that are still being investigated, he went out of bounds and when trying to come back he overturned due to the 50 cm drop. The driver left the cab under his own power and reported the incident. Holcim staff rushed to the scene of the incident and took the driver to a clinic for medical attention. Why? • Dark environmental conditions. • Complacency overconfidence collaborator. • Distraction factor having received calls even though you don't answer the phone. What we learned • That self-identification of mental states is important to trigger alertness. • The fact that it is important to measure the frequency of risk analysis (2-2=0) is an aspect that has not yet become a habit in most collaborators. • That there are distractors such as a call entry that, even if the driver does not answer the phone, can become a factor of distraction and risk. What I will do • Carry out technical stoppages in plants to publicize the incident. • Continue promoting the safe habits and group training program and the identification of mental states through risk analysis in the field • Promote the APP application so that collaborators can record their risk analysis in the field and measure the frequency and habit of analysis and establish an indicator of the number of analyzes carried out by each collaborator. • Prepare the talk of distractors when driving and highlight the unforeseen aspects and carry out an evaluation to ensure that it has been understood. • Make balance of consequences to collaborator.
