Safety Moment – Lessons Learned of Lifting Accident

 Incident Overview
 On June 3, 2022, one of WISON subcontractors, carried out the installation of the 25-meter high pole street lamp
on the road west of the acrylic tank group. Before lifting, the high-pole lamp is placed horizontally on the ground.
The light pole is thicker at the bottom and thinner at the top, 24 meters long and weighs about 3 tons. The rigger
Mr. Gao and the helper Mr. Li was working for the lifting with a 25-ton crane, at the same time, the lamp
manufacturer service personnel , Mr. Zhao and Zhou, was guiding the installation on site.
 At 14:00, before lifting, Zhao and Zhou , the on-site service personnel of manufacturer, connected an 8-meter-long
steel wire rope and two 8-meter-long 5-ton web slings with two U-shaped lifting shackles. The wire rope was fixed
at the bottom of the lamp, and the web sling is wound around the uppermost light pole about 4 meters from the top
and fixed in a knot, the rings of the sling are directly hung on the hook of the crane for lifting.
 At 14:10 the lifting started. When the light pole was close to
the vertical angle and was ready to be put in place on the
foundation, the upper sling suddenly broke, and the light
pole lost its balance and fell to the north side of the spherical
tank T-50201.
 The accident caused deformation and damage to the lamp.
Some of the facilities near the spherical tank damaged as
well. The accident caused no injuries.

 Causes of incident
 Direct cause: The fracture of the sling was the direct cause of the accident. The upper sling is
directly wrapped around the uppermost light pole and then hung on the hook. There is no
master link assembly attached to the web sling. During lifting, the web slings and the wire
rope are gradually tightened upwards, the sling is subject to tension and a shear force caused
by the friction of the sling itself. When the light pole is close to the vertical state, the shear
force received to the maximum value at the knot point, causing the sling Suddenly broke.
 Root causes:
• 1. The rigging method for lifting was wrong, and the rigger did not perform his due duties.
The rigger Gao did not Inspection and Evaluation before lifting. Instead, the ring was directly
wrapped around the lashing. Gao, as the lifting conductor, should have recognized the wrong
method, but he did not fulfill his duties and did not correct the wrong method in time.
• 2. The safety assessment before lifting was not carefully carried out. Subcontractor did not
develop a lifting plan for this operation; the manufacturer's on-site service personnel only
operated based on existing experience.
• 3. There are loopholes in the management of personnel and vehicles entering the venue. The
post-accident investigation showed that the crane operator did not have the valid certificate
issued by the authorized department, and did not participate in the project’s induction
training, he did not fill in the "Crane Daily Inspection Form" .

 Corrective Actions
 1. Learning the lessons of the accident, the Project HSE team communicated the accident with the safety
directors of each construction unit in the afternoon of the same day, asking each unit to take precautions
and strengthen the management of lifting operations.
 2. On the second day after the accident (June 4th), the Project Department issued the "Notice on
Strengthening the Management of On-Site Hoisting Operations" (919181-PI0613-H00-115) and put
forward relevant requirements.
 3. On the morning of June 5, a Stand Down was organized to all subcontractors.
 4. The concerned subcontractor is required to
develop the lifting plan for high pole lights, and
the subsequent installation can only be carried out
after approval.
 5. The Project department further strengthened the
safety management of key links such as lifting
operations, certificates for employment, personnel
entry training, and work permit management.

 Strengthen process inspection and control of lifting ensure lifting operation following lifting standard and
regulations
 Interview to the lifting team
 Inspection and confirm work permit, MS and JSA, lifting plan before lifting
 Daily inspection to the crane, lifting gears and construction facts before lifting
 TBM of lifting team before lifting
