OBJECTIVE

 Tolearn the application and benefits of Lock-Out


& Tag-Out Systems.
INTRODUCTION 4

 Rotating Machinery, Circuit Breaker or energized


equipment must be isolated from all energy sources
before beginning any repair or maintenance in
order to prevent serious accident, fatality or
property damage.
 Energy stored, hazardous energy shall be relieved,
disconnected, restrained or secured to make the
equipment safe. Energy includes electricity, steam
or pressurized fluids, suspended loads, or elevated
or rotating loads.
DEFINITIONS 5

 Lockout: the placement of a lock on energy


isolating device ensuring the equipment
cannot be operated until the lock is
removed.
 Tagout: the placement of a tag on an
energy isolating device ensuring the
equipment may not be operated until the
tag is removed.
WHY IS LOTO IMPORTANT?

To protect workers from the increased risk


of injury of exposure to the unintended or
inadvertent release of energy when safety
devices are removed during non-routine
activities such as:
 Maintenance
 Repair
 Set-up
 Removal of jams, clogs or misaligned feeds

Workers servicing or maintaining machines or


equipment may be seriously injured or killed if
hazardous energy is not properly controlled.
Injuries may include:
 Electrocution
 Burns
 Crushing
 Cutting
 Lacerating
 Amputating
 Fracturing body parts, and others

OUT OF SERVICE TAG APPLICATION


 Use when operation may endanger personnel
or plant equipment

 This Tag shall be securely attached to the following


(Defective):
 Light and Heavy Equipment
 Utilities
 Hand Powered Tools
 Devices
 Machines, etc.

 To be removed only by the person who attached it and
until rectification!!!

PERSONNEL DANGER TAG

 Usedwhen person(s) will repair or work on


defective equipment, etc.

 To
give notice or warning that someone is
working on the system.

 To be removed only by the person who attached


it and until work has been completed.

 Padlocksshall be used to ensure that no one can


open &/or switch-on the system.

 Sometimes, Out of Service Tags & Personnel


Danger Tags are being neglected, so padlocks
will protect.

 With only one key.

RESTORING EQUIPMENT &


ENERGY SOURCES
 After servicing or completing maintenance, make
sure the equipment is ready for normal operations.
 Check the equipment and energy sources to ensure
the area is safe.
 Make sure the guards are re-installed and personnel
are clear of danger.

INSTRUCTIONS
 Provide Tag-Out & Lock-Out Cabinet.
 Obtain these accessories from maintenance
or HSE Dept.
 Only standard Tags must be used.
 Removing or tearing of Tags without
authorization will be ground for serious
disciplinary action.

properly displayed.
 When more than one
lock will be attached, a
multiple lock adapter
will be used.
Application of 17

Lockout/Tagout Devices
a) Make necessary preparations for shutdown.
b) Shut down the machine or equipment.
c) Turn OFF (open) the energy isolating device
(fuse/circuit breaker).
d) Apply the lockout or tagout device.
e) Render safe all stored or residual energy.
f) Verify the isolation and de-energization of the
machine or equipment.

Devices
g) Inspect the work area to ensure that nonessential
items have been removed and that machine or
equipment components are intact and capable of
operating properly. Especially look for tools or pieces
of conductors that may not have been removed.
h) Check the area around the machine or equipment to
ensure that all employees have been safely positioned
or removed.
i) Make sure that only the employees who attached the
locks or tags are the ones that are removing them.
j) After removing locks or tags, notify affected
employees before starting equipment or machines.
