Primary impression
Why heart failure
• Symptoms- dyspnea, ankle swelling and orthopnea, neck vein
engorged
• HR- 122
• In respiratory distress
• Signs- bipedal edema, elevated neck veins, diastolic murmur and 3/6
holosystolic murmur and 3/6 systolic murmurs.
• NYHA class IV
Jones criteria
• Major manifestation- carditis
• Poly arthritis
• Chorea
• Erythema marginatum
• Subcutaneous nodules

• Minor manifestations- fever, arthralgia, increased ESR/CRP, prolonged PR


interval.
• For rheumatic fever – 2 major manifestations/ 1 major or 2 minor
manifestations.
