DESCRIPTION OF THE WORKPLACE INCIDENT, IDENTITY OF THE
RELEVANT AUTHORITY THAT SHOULD ATTEND TO THE INCIDENT
AND AN EXPLANATION OF THEIR ROLE
• A 59-year-old worker fell into the sea • Bosses of companies are responsible-
while crossing a gangway and drowned they should make sure that workers wear
• The location of the accident that took safety wear such as a life jacket so they
will not drown

STATING AND ELABORATION OF 3 FACTORS
THAT CONTRIBUTED TO THE INCIDENT
• Lack of risk assessment- There was no proper evaluation of the gangway and the risks
involved in crossing it, and if it was safe for workers to cross it
• Failure to comply with safety and health regulations- Alliance Instrumentation did not
check the width of the gangway
• Lack of workplace safety measure- workers were not being briefed about the dangers of
crossing the gangway
HOW IT COULD HAVE BEEN BREACHED DURING THE INCIDENT
• Lack of risk assessment- Alliance instrumentation did not consider the dangers of
crossing the gangway
• Failure to comply with safety and health regulations- the workers did not look at the
width of the gangway and just crossed the gangway.
• Lack of workplace safety measure- the workers did not know what was safe and what
was not safe.
SUGGESTION AND EXPLANATION OF TWO PREVENTIVE
MEASURES TO AVOID A SIMILAR WORKPLACE INCIDENT
• Provide safety training to ensure that the workers are educated of the dangers and risk of
each worksite and avoid those areas.
• Conduct thorough risk assessment to evaluate the risks of crossing the gangway.
