ECG and Arrythmias

Normal ECG
1. Rate
2. Rhythm
3. P-wave
4. Axis
5. PR interval
6. QRS
7. ST segment
Arrythmias
SVT
Atrial flutter
Atrial flutter
Atrial fibrillation
Monomorphic VT
Ventricular fibrillation
Hyperkalemia
Hypokalemia
• Increase P wave amplitude
• Prolonged PR
• T-wave flattening/inversion
• U wave
• Prolonged QT
Myocardial infarction
Anterolateral wall STEMI
Inferior wall STEMI (Deadman sign)
NSTEMI
ST elevation in avR
• Triple vessel disease
• Proximal LAD obstruction
• Posterior wall STEMI
• After resuscitation from cardiac arrest
• Treatment: Rescue PCI
Heart block
First degree Heart block
Mobitz type 1
Mobitz type 2
Third degree heart block
Left bundle branch block
• QRS wide
• V1: deep and broad S with no R
• Lead 1 and V6: broad slurred R without Q
• Always have left axis deviation
Sgarbossa criteria
Right bundle branch block
• QRS wide
• V1: RSR
• Lead 1 and V6: broad and slurred S wave
• Right axis deviation may or may not present
Left ventricular hypertrophy
• Sokolow-Lyon criteria:
• S in V1+R in V5 or V6= 35mm
• Or
• R in V5 or V6>26
Right ventricular hypertrophy
• In V1: R wave> S wave
• R in V1+ S in V5 or V6= 11mm
