CATARACT

Definition

It is one of the most common eye disorders


A cataract is a lens opacity or cloudiness.
Lens appear gray or milky white.
Classification
1. Nuclear catract
It is caused by central opacity in the lens and has a
substantial genetic component.
2. Cortical Catract
it involves the anterior, posterior, equatorial cortex
of the lens
3. Posterior Subcapsular cataract
It occures in front of the posterior capsule.

Sunlight and ionizing radiation Secondary effects- Uveitis, glaucoma, retinitis

obesity Prednizone, Ergot alkaloids
Eye injury Dinitrophenol, naphthalene
High tri glyceride level in men Phenothaziphines or pilocarpine
Myopia Extended exposure to ultraviolet rayes
Older age,Arthritis, heart disease
Clinical Manifestations
1. Blurred vision
2. Glare-pain felt when the patient see the light
3. Halos- when looked at bright light and still the
vision of the light after looking away
4. Double vision-Diplopia
Prevention
• Quit smoking
• Wear Sunglasses

Complications

Reetrotubular hemorrhage
Acute bacterial endophthalmitis
toxic anterior segmant syndrome due to infection and inflammation of anterior chamber
Diagnostic Evaluation
1. Snellen visual acuity test
2. Opthalmoscopy to analyse the exent of cataract
3. Slit lamp biomicroscopic examination to establish
the degree of cataract formation.
Medical management
Dilating drops- administered every 10 mts atleast
for one hours.
Antibiotic drugs- prophlactically to prevent
infection
Intravenous sedation- to minimize anxiety
Surgical management
• Lens replacement surgery
• Phacoemulsification- a portion of anterior capsule
is removed,allowing extraction of the lens neucleus
and cortex while the posterior capsule and zonular
support are left intact.
• Aphakic glasses
• Contact lenses
• IOL implants
• Extra capsular cataract extraction (ECCE)
• Intra capsular cataract extraction
Nursing diagnosis
1. Disturbed visual sensory perception related to
altered sensory reception or status of sense
organs
2. Risk for trauma related tompoor vision and reduce
hand eye co ordination
3. Anxiety related to threat of permanent loss of
vision/ independence.
4. Deficient knowlwdge regarding altered coping
abilities related to cognitive and perceptual
limitations.
