WORKPLACE PROCEDURE
Is a set of written instructions that
identify the health and safety
issues that may arise from the jobs
and tasks that make up a system
of work.
The safe working procedure should
identify:
The teacher for the task or job the
students who will undertake the
task.
The tasks that are to be
undertaken that pose risks.
The equipment to be used in these
tasks.
The safe working procedure should
identify:
The control measures that have
been formulated for these tasks.
Any training or qualification needed
to undertake the task.
The safe working procedure should
identify:
The personal protective equipment
to be worn.
Action to be undertaken to address
safety issues that may arise while
undertaking the task.
COMPONENTS OF
WORKPLACE
SAFETY
PROCEDURES
1. Observe the task/activities
It is important to observe the
task/activity being performed in
the preferred way to ensure the
safest method is documented.
2. Review associated legislative
requirements.
Some tasks/activities are
governed by legislative
requirements.
3. Record the sequence of basic job
steps.
Write down the steps that make
up the task/activity.
4. Record potential hazards of each
step.
Next to each step identify what
may have potential to cause injury
or disease.
5. Identify ways of eliminating and
controlling the hazards.
List the measures that need to be
put in place to eliminate or control
any likely risk.
6. Test the procedure.

Observe staff/student following the


safe work procedure.
7. Obtain approval.

Before the safe work procedure


can be used it must be approved
by each approver nominated.
8. Monitor and review.

Make sure the activity is


supervised to ensure the
documented process is being
followed.
WORK-RELATED
ERRORS
2 MAIN TYPES OF HUMAN FAILURE:

1. Human error – is an


unintentional action or decision.
2. Violations – are intentional
failures – deliberately doing the
wrong thing.
1. HUMAN ERROR
These types of human error can
happen to even the most
experienced and well-trained
person.
A. SKILL-BASED ERRORS

Slips and lapses – occur in very


familiar tasks which we can carry
out without much conscious
attention.
Example of SLIPS:
(Not doing what you’re meant to do)
1. Performing an action too soon in
a procedure, or leaving it too late.
2. Omitting a step or series of steps
from the task
Example of SLIPS:
(Not doing what you’re meant to do)
3. Doing the right thing but on the
wrong project.
4. Performing an action in the
wrong direction.
Example of LAPSES:
(Forgetting to do something, or losing your
place midway through a task)

1. Forgetting to continue the task


because of the interruption.
2. Takes time to finish the task
and try to remember the next
step.
B. MISTAKES

Are decision-making failures.


The 2 main types of mistakes are
rule-based mistakes and
knowledge-based mistakes.
Example of MISTAKE:

1. Misinterpret the sign and symbol.


2. Believing the wrong
measurement of the defective
instrument.
2. VIOLATIONS

These are intentional failures –


deliberately doing the wrong
thing.
Example of VIOLATIONS:

1. Not wearing PPE inside the


worksite.
2. Smoking inside the workplace.
