1.9 Mandatory Occurrence Reporting Cont.
Any part of the aircraft that would endanger the aircraft or any person by becoming
detached in flight or during operations on the ground.
Aircraft structure that requires major repair;
Cracks, permanent deformation, or corrosion of aircraft structure, if more than the
maximum acceptable to the manufacturer or the Authority;
Uncommanded loss of thrust/power, shutdown or failure of any engine;
Uncontained failure of engine compressor, turbine;
A propeller feathering malfunction or inability of the system to control over speed
during flight;
Fire or explosion;
An aircraft component that causes accumulation or circulation of smoke, vapour, or
toxic or noxious fumes in the crew compartment or passenger cabin during flight;
Fuel leakage that results in substantial loss or is a fire hazard;
Fuel or fuel dumping system failure that affects fuel flow or causes hazardous
leakage during flight;
Fire warnings, except those immediately confirm as false;
An unintended landing gear extension or retraction, or opening or closing of landing
gear doors during flight;

Brake system component failure that results in loss of brake actuating force when
the aircraft is in motion on the ground;
Any loss or malfunction of one or more main system(s), subsystems, or set(s) of
equipment (e.g. hydraulic power, flight controls system [auto flight, auto trim],
electrical power, air system, ice protection, navigation systems and instruments,
warning systems and devices, brake systems);
Fires during flight and whether the related fire warning system properly operated;
Fires during flight not protected by a related fire warning system;
False fire warning during flight;
An engine exhaust system that causes damage during flight to the engine, adjacent
structure, equipment or components;
Any failure, defect, malfunction or deterioration of any critical item, system, or
equipment found as the result of any special mandatory inspection or check (e.g.
an airworthiness directive or alert service bulletin);
Defects or deterioration of systems or components found during routine
maintenance, overhaul or repair when of a type not expected as a result of normal
service;

The number of propeller featherings in flight, listed by type of propeller and engine
and aircraft on which it was installed.
Ethiopian Airlines is responsible to ensure that each report to ECAA: Shall be made
within 3 days after determining that the failure, malfunction, or defect required to be
reported has occurred; and
Shall include as much of the following information as is available and applicable:-
Aircraft serial number;
When the failure, malfunction, or defect is associated with an article approved
under a TSO authorization, the article serial number and model designation, as
appropriate;
When the failure, malfunction or defect is associated with an engine or propeller,
the engine or propeller serial number, as appropriate;
Product model;
Identification of the part, component, or system involved, including the part number;
and
Nature of the failure, malfunctions, or defects.
