What is an
inversion ankle
sprain
• This is an injury that occurs when
the foot turns inward to an
abnormal degree relative to the
ankle
Ankle anatomy
Anterior Talofibular Ligament (ATFL)
Fibula
Calcaneus CFL
Posterior Talofibular Ligament (PTFL)
Fibula
PTFL
Talus
Mechanism of injury
• Inversion of foot
• Plantar flexion + Inversion
• Velocity and angle of plantar flexion determines severity of the injury.
Signs and symptoms
• Crack/pop sound
• Pain during when weight bearing
• Pain upon palpation of ankle joint
• Swelling
• Bruising
• Restricted range of motion
• Ankle instability
• Deformity
Appropriate management
• Evaluate range of motion: Active, passive, resisted
“there will be pain on active and passive movement in same direction”
Resisted motion grading
You want to know how much strength the injured foot has
0 = no movement
1 = Muscle twitch
2 = Antigravity
3 = Move against gravity
4 = withstanding resistance
5 = Full strength
Special tests
Anterior and posterior drawer test & Talar tilt test
