    1) A chemical spillage at a dyeing factory was identified as the main problem. Root causes were identified as worker skills issues, lack of monitoring by production executives, and overloading of chemical transport trolleys. 2) Analysis using a fishbone diagram and 5 whys technique traced the causes to lack of worker awareness and training, insufficient oversight by management, and damaged trolley wheels. 3) A control plan was created to improve worker training, increase management monitoring, and replace damaged trolley wheels on a monthly basis. The goal is to reduce defects from manufacturing processes.
