History :
20 years old male presented with Sudden onset
shortness of breath while playing football.

Clinical findings:
Tachycardia ,hypotension and distended neck veins

Radiological Findings

PA Chest X ray (A) shows:


• Increase translucency of the
peripheral right hemithorax
• Absent broncho vascular
markings in the right
hemithorax
• Deviation of the heart and
mediastinum to the left side
• Collapsed right lung
• Opacified right CPA

Post insertion of a right
pleural pigtail catheter
>>>> resolution of tension.

Differential Diagnosis:
1-Pneumothorax
2-Malrotated film
Diagnosis:
Right Tension pneumothorax with mild pleural effusion
( Hydropneumothorax )
Treatment:
1-Urgent decompression by insertion of chest tube.
2-Surgical consultation
Discussion:
• Pneumothorax is the collection of air within the pleural space.
• It is classified as spontaneous (the most common), traumatic, or
iatrogenic.
• It is common in male.
Causes of pneumothorax include:
• Chest injury
• Lung disease
• Ruptured air blisters
• Mechanical ventilation
Radiographic features of Pneumothorax:
Pneumothorax usually appears on Chest X ray
as:
• Radiolucent region with no lung markings
peripheral to the white line of the visceral
pleura. (blue line ).
• Usually occur in the apices (The air rises to
the least dependent regions.
• Rib fractures can be identified in post
traumatic cases .(arrow)
• Pneumothorax is demonstrated better by an
expiratory film.
Types of pneumothorax
• Closed pneumothorax: air retained in the pleural space.
• Open pneumothorax: the air can enter to and exit from the pleural
space.
• Valvular pneumothorax: the air can enter to and can't exit from the
pleural space causing progressive increase intra-pleural pressure and
leads to tension pneumothorax
Tension pneumothorax: is the worst type that may be fatal by
compression of the heart causing cardiac tamponade.
Treatment of pneumothorax:
Tension pneumothorax is an
emergency case that needs:
1-Urgent decompression by
insertion of chest tube.
2-Surgical consultation
