SEPTOPLASTY
 It is a conservative approach to septal surgery.
 It is done to correct the deviation of nasal septum.

GOALS :
 improve breathing through nose,

 prevent sinus infections.


INDICATIONS OF SEPTOPLASTY
 Nasal airway obstruction
 Chronic nose bleeds

 Headaches associated with nasal spur

 Recurrent epistaxis due to nasal spur.

 Sinusitis due to deviation of nasal septum

 Obstructive sleep apnea

 Cosmetic purpose
CONTRAINDICATIONS
 Acute sinusitis
 Nasal infection

 Untreated diabetes

 Hypertension

 Bleeding disorders
TYPES OF INCISIONS
 Killian’s
 Transfixion

 Hemi transfixion

 Horizontal on spur
PROCEDURE
 Anesthesia: local or general anesthesia
 An incision is made in the lining of the septum

STEPS
 Infiltration

 Incision: Freer's incision-a unilateral hemi-transfixion


incision at the caudal border of the septum to reach the
cartilage.
 Exposure: the mucoperichondrial and mucoperiosteal
flap is elevated on only one side.
 The septum may then be stabilized with small plastic
tubes or plints.
 Separate septal cartilage from vomer and ethmoid plate
 Inferior strip of cartilage is removed

 Correct the bony septum by removing deformed parts

 Minor deviations of cartilage are corrected by criss cross


incision which breaks spring action of cartilage
 Nasal packing
POST-OPERATIVE CARE
 Patient should drink and eat soft food
 Avoid all moderate physical activity for couple of days
after operation
 Avoid bending

 Avoid smoky, dusty and dry atmospheres

 Saline spray or steam inhalation are suggested after pack


removal
 Xylo or Oxymetazoline drops or used if nose is stuffy
COMPLICATIONS
 Bleeding
 Septal hematoma

 Septal perforation

 Damage to surrounding structure

 Retraction of columella

 Infections like Sinusitis and Midilear infections

 CSF rhinorrhoea

 Toxic shock syndrome
