Proposal for the System Function Complying Revision

• Proposed PTW System Function.

• ENERFLEX PTW Coordinators will be assigned to manage the PTW System:

• Construction Area - One PTW Coordinator (with back-to-back) + PTW Officer/Administrator (with back-to-back)
• Commissioning Area -One PTW Coordinator (with back-to-back) + PTW Officer/Administrator (with back-to-
back)

• Dedicated PTW Offices - one per each Construction and Commissioning area.

• Offices will be open for permit verification and registration (Proposed Timings are 06:30 to 11:15 Hrs) and
signing session (Afternoon 16:30 to 17:15)

• Permit forms will be made available from the PTW offices and respective contractor PTW Coordinators
/Administrator will collect, keep and control their issue.


Signing Session of Permits/Meetings

• Proposed Signing System

• Enerflex Area PTW offices are Open for Contractors from 06:30 to 11:15 Hrs and on Signing Session
(time to be agreed).

• Permit requestors visit their respective Area PTW offices during this period to undergo permit review
and registration.

• There will be only 1 (ONE) Signing Session Meeting for the Construction area which is proposed to
happen from 16:30 Hrs to 17:15 Hrs at Offsite SITE OFFICE

• The IA / AA /Permit requestor Contractor/Permit Coordinators are to be present in the Signing


sessions.

Step 1

PA or Contractor PTW Coordinator/Administrator (having detailed knowledge of the task and


trained in ENERFLEX PTW System) will visit the ENERFLEX Area PTW Office from 06:30 to
11:15 Hrs to raise a permit.
The Risk Assessment/Method statement/Drawings and any other required
attachments/documentation are to be provided with the permit request.

PA ENERFLEX PTW Coordinator reviews and inspects the request for a permit and relevant
documentation.
The review process is complete, It OK for the permit to be raised

ENERFLEX PTW Office now will collect the permit (registering takes place by each
Contractor PTW office)

Permit requestor and Performing Authority shall not sign in advance permit PA section

Step 2

PA/Permit Requestor or Contractor PTW Coordinator /Administrator takes the permit to the
respective AA) to discuss the task and to review any required certificates, documentation, and
controls to be implemented.

Step 3

PC forwards the Permit to the respective AA on the field for his review via Contractor
PTW Coordinator/Permit requestor
The permit is with AA. (for critical activities)

Step 4

AA checks the worksite and ensures all the precautions can be implemented and agrees to make
regular checks at the work site to ensure permit requirements are met. AA reviews any conflicts with
previously raised permits/works.

AA keeps the permit and brings it to the Permit office.

AA is to coordinate the work with Affected Parties (if any) in between.

Once the AA is satisfied, permit will be addressed to the IA review and approval at the Signing Session.

Any permits not accepted by AA shall be returned to IA at the signing session for rectification or
cancellation.

Step 5
IA reviews the Permit / Supporting documents at the daily Signing Session
subject to AA review being completed.

IA Signs the Permit – FIRST SIGNATURE.


AA
AA collects the permits from IA.

Any not approved permits are returned to PC.

If Affected Parties are involved, they sign in at the signing session following the
AAA signature of the IA.

Step 6

AA at his carefulness signs the permit either at the Signing Session or at the site on the following
day.

If AA decides to sign a permit just before the work commencement and his personal site visit and
verification of the contractor’s preparedness, he will withhold the permit and sign it at site on the
AA following day.

AA Signs the Permit – SECOND SIGNATURE.

Once signed, a permit is handed over to the PC, and PA comes to Permit Office and Sign the permit

Step 7

PC to be present at the Signing session.

PC collects the permit from ENERFLEX AA (if signed at the signing session).

PC confirms understanding and agreement to implement all required controls as per the permit
IA conditions.

PC transfers the permit to PA and arranges for any required isolations and gas tests if required by
permit conditions.

Step 8

PA accepts and signs the Permit from PC, before commencing the work conducts TBT and pre-
task briefing of the crew on the hazards and control measures to be adopted.

PA Briefing and TBT completed by PA.

PA Signs the permit - THIRD SIGNATURE

Step 9

AA or his delegate visits the site before work commencement to ensure all the permit requirements
are met by PA.

AA The permit will become live only when the day’s endorsement is done by all (AA,& PA)
PC retains Copy # 2 and PC updated the PTW database for the Issued and Approved Permit

Step 10

PA to ensure PTW Copy # 1 is displayed at the site.

Once the work is finished / permit validity expires / permit is suspended for any reason, the PA must
make the worksite safe or initiate the revalidation process for the permit.
PA PA Signs the Permit and Hands over the permit to PC (Permit office at the end of the shift)
If the permit is suspended for any reason PA MUST IMMEDIATELY inform the PTW Office and
respective IA and AA

Step 11

The AA visits the site on completion of the task to verify the site is left in a safe
condition.

AA will close the Permit by signing it.

PTW Training
• Who needs to be trained – to be evaluated.

• Training requirements for various personnel:

• IAs; AAs – Refresher training and introduction to PTW Rev. – Max 1 hours
• PA’s – Max 2 Hour custom refresher training and introduction to PTW Rev.
• Newcomers – 2-hour full PTW/TRA Training on Rev.
• Contractor PTW Coordinators* – Full refresher training + 2-hour Workshop with Enerflex PTW Coordinator

• * Competency criteria will be applied for this position prior to acceptance.

• GENERAL NOTE:

• The PTW courses can be modified in consultation with senior management to suit particular site needs
and requirements.
Other Points To Emphasis
• Always try to take the permit for the minimum number of days required for the task with
minimum buffer days (Max 07 days) as it is observed in many cases the permits are
issued for 7 days only even if task can be finished in 2 to 3days.
• IF The Task is Finished before attaining the max validity period, return the permit to
Area PTW Office Immediately

• Remember there will be a limitation on the number of permits, that can be “SAFELY
ISSUED AND HANDLED IN EACH AREA” (Presently the system is under monitoring to
implement the limitations )

• Take utmost care While filling up and handling Work permits & Certificates. “
Overwriting or using correction fluid invalidates any permit or certificate resulting in “
IMMEDIATE CANCELLATION” (any mistakes or errors has to be crossed out with a
SINGLE LINE and INITIALLED by the IA& AA)
