Incident Description:
While repairing a wheel loader on the main road near the MGP plant, there was a spill of
100ml of diesel directly onto the asphalt road due to inadequate maintenance procedures and
the absence of a dip tray to contain the spillage. immediately suspended the activity, Graphic on what went wrong.
cordoned off the area, and cleaned up the spill according to the OQ procedure.
