5.3 Functions and Responsibilities

1. Provincial Functionality Assessment Focal Person


The Provincial Functionality Assessment Focal Person shall be in charge of:
a. Orienting the C/MLGOOs in the conduct of the functionality assessment;
b. Collecting the required data from the Provincial Monitoring Committee by accomplishing
the LGU Document Checklist (Form 1) at the provincial level.

2. Provincial Functionality Assessment Team (PFAT)


c. The team shall evaluate the functionality of the Local Project Monitoring Committee of
the Cities and Municipalities using the Assessment Tool (Form 2) based on the
submitted LGU Document Checklist (Form 1) from the CMLGOOs.
d. The team shall also consolidate and submit the results of the functionality assessment
using the Consolidated Assessment form (Form 3) to the DILG Regional Office.
5.3.3 Highly Urbanized City (HUC)
Director
a. The HUC Director shall designate an HUC
Functionality Assessment Focal Person.
i.The HUC Functionality Assessment Focal
Person is in charge of collecting the required
data from HUC PMC by accomplishing the
LGU Document Checklist (Form 1) for the city.

ii.The HUC Functionality Assessment Focal

5.3.4 DILG Regional
