INTRA-UTERINE CONTRACEPTIVE

DEVICE(IUCD)
OBJECTIVES
AT THE END OF THE PRESENTATION LEARNERS SHOULD BE ABLE TO:
-Describe IUCD
-Tell Mode of action of IUCD
-List Advantages and Disadvantages of IUCD
List the side effects of IUCD
• It is a flexible device which may be plastic or
metal inserted into the uterus by a trained
health personnel.
• It is inserted via the cervical canal and have
threads attached which are visible at the
external cervical os .
TYPES OF INTRA-UTERINE DEVICE
• The hormonal IUD
• The copper IUD

NB : An is IUD highly effective and reliable . They


last for 5years or more.
THE HORMONAL IUD
• They are made of plastic and slowly release a
progesterone hormone called LEVONORGESTREL.
• The hormonal IUD prevents pregnancy by;
-thickening the mucus at the entrance to the uterus
so that sperm cannot get through.
-making it harder for sperm to reach and fertilize an
egg.
-thinning the lining of the uterus.
-preventing ovulation in some people.
The copper IUD
• Made of copper and plastic . It does not release
any hormone.
The copper IUD works by;
-making it harder for sperm to reach and fertilize
an egg.
-making it harder for an egg to survive.
-changing the lining of the uterus so that if an egg
was fertilized , it would not be able to implant.
MODE OF ACTION
Copper based IUCD release copper and prevents
pregnancy by a combination of mechanisms of action
which include:
-Inhibition of sperm migration in the upper female
genitalia.
-Inhibition of ovum transport.
-Inhibition of fertilization.
In addition , the levonorgestrel - releasing IUCD causes
changes in the amount and viscosity of the cervical
mucus thus inhibiting sperm penetration.
ADVANTAGES
• They are extremely good at preventing
pregnancy.
• They can be used if a person is breastfeeding.
• IUDs are cost-effective.
• Long duration of use.
• It can be inserted immediately after an
abortion.
• It usually does not affect mood or sex drive.
DISADVANTAGES
• It offers no protection against STIs and HIV.
• There is a small risk with insertion such as
difficulty with insertion , pain or discomfort ,
damage to the uterus and infection.
• The woman cannot stop using an IUCD on her
own.
• Risk of ectopic pregnancy.
SIDE EFFECTS OF IUCD
• Irregular periods
• More or worse cramping during periods.
• Heavier or longer periods
• Breast tenderness
• Headache
• Anaemia due to blood loss.
FOLLOW-UP
• The client should be given the date of the next
visit , information about the name and type of
IUCD she has obtained and when it should be
removed or replaced .
• If possible,all relevant information, the list of
warning signs , the name , address and
telephone number of the clinic should be put
on a card or leaflet and given to the client .

• During follow-up care at 3 months and
annually;
-Update the client`s address and how to make
contact with her.
-Discuss with the client any question , concern
or problem she may have related to the method.
-Perform a vaginal examination and review
instructions for checking the IUCD threads.
