ANC Hx
• G4 P3 A3 booked at 20 weeks and attended 4 antenatal con-
tacts.
• HIV status non reactive, obstetric scan done which revealed
Breech presentation during ANC,

AT PUC
• Mother reported at PUC at 04:48 of 11/10/2023
• with complaints of lower abdominal pains since 19:00 of 10/10/2023
• There was no fetal heart heard on admission
• She was then referred to PDH due to no fetal heart in breech presen-
tation.

AT PDH
• came as a referral from PUC at 05:30 of 11/10/2023 due to
breech with no fetal heart heard.
• on admission BP 106/74 PULSE 84, respiration 20,
temperature 36.6 URINALYSIS: indicate the results for
urinalysis one by one, not All parameters normal.
• Cervical Dilatation: OS was fully dilated with no fetal heart,
contractions 3:10 moderate.

At 05:34hrs a fresh still birth male infant was delivered via SVD
btw 2800g.
Gaps identified

• mother delayed to seek health care: she reported to the health facility
in established labour with malpresentations (breech).
• delayed referral from the health facility: should have been referred for
hospital delivery during antenatal.
Recommendation

• Intensify community IEC on the importance of early seeking of health


care services.
• ensure all high risk pregnancies are identified and referred using a
national maternal referral form for hospital delivery during antenatal.
