NON-RENAL ROUTES OF DRUGS EXCRETION

Drugs and their metabolites may also be excreted by routes other than the renal
route, called as the extra renal or non-renal routes of drug excretion.

1. Salivary excretion.
2. Mammary excretion.
SALIVARY EXCRETION :-

Passive diffusion process.

Based on pH partition hypothesis.

pH of saliva is 5.8 to 8.4.

Unionized, lipid soluble, soluble drugs at this pH are excreted passively.

S/P ratios is <1 for weak acid & >1 for weak bases.

Basic drugs secreted more in saliva, eg. caffeine theophylline etc.

Some drugs are actively excreted in saliva, eg. Penicillin, phenytoin etc.

Bitter test in mouth is an indication of drug excretion in saliva.

These process are responsible for side effects such as….

i. Black hairy tongue (Antibiotics)

ii. Gingival hyperplasia (Phenytoin)

iii. Some basic drugs inhibit saliva secretion and are responsible for dryness of mouth.
MAMMARY EXCRETION :-

Important since it can gain entry in to breast feeding infant.

Passive process, dependent on pH partition behavior, molecular weight, lipid


solubility and degree of ionization.

pH of milk varies from 6.4-7.6 (mean pH – 7.0)

Since milk is acidic in comparison to plasma, weakly basic drug concentrate


more in milk.

E.g.:- atropine, antihistaminic, metronidazole.

The amount of excreted in milk is generally less than 1% and fraction
consumed by infants is too less to reach therapeutic or toxic levels.

But some potent drugs such as barbiturates, morphine may induce toxicity
in infants.

 Chloramphenicol : possible bone marrow suppression.

Diazepam : accumulation and sedation.

Heroin : prolonged neonatal dependence.


Tetracycline : permanent staining of infant teeth.
