• Defined by the DOH DDAPTP Central Office and signed by the
DOH Centers for Health Development- Regional Offices
• Participants can only receive a Certificate of Completion if they
participated in ALL of the activities during the training.
• Tardiness and/or missing a session will only guarantee a
Certificate of Appearance.
• Post test score of 80%
