Introduction

• Vitamin D deficiency is a major public health


problem in India, affecting all age groups, but
especially children and pregnant women.

• Milk is a staple food in India, and fortifying it


with vitamin D3 is a promising strategy to ad-
dress this deficiency.

Vitamin D3 Deficiency in the Indian Population

 Vitamin D deficiency is a major public


health problem in India, affecting an es-
timated 70-80% of the population.

 The prevalence of vitamin D deficiency is


highest among children and pregnant
women, with 92% of children under 5 and
75% of pregnant women being deficient.

 Vitamin D deficiency is also common


among adults, with an estimated 68% of
adults being deficient.

Why Milk Fortification?
Overall, fortification is the preferred method of vitamin D3 supplementation because it is more cost-
effective, reliable, sustainable, and equitable. In addition, milk fortification is a more equitable way
to improve the vitamin D status of a population. It ensures that everyone has access to vitamin D,
regardless of their income, education level, or other factors. Which can not be achieved through
health supplementation distribution through government.

The study was conducted by researchers from the National Institute of Nutrition in Hyderabad, In -
dia. They enrolled 400+ children in the study and randomly assigned them to one of three groups:

 Group A received unfortified milk.


 Group B received milk fortified with 100 IU of vitamin D3 per 100 ml.
 Group C received milk fortified with 200 IU of vitamin D3 per 100 ml.
 The children were followed for 12 weeks. At the end of the study, the researchers found that
the children who drank fortified milk had a significant increase in their vitamin D levels, com-
pared to the children who drank unfortified milk.

Current Scenario of Milk Fortification
in India
• Milk fortification with plant-based vitamin D3 is still in its early stages in In-
dia. Very few milk brands are currently fortified with plant-based vitamin D3,
and there is a lack of awareness among consumers and healthcare profes-
sionals about the benefits of fortified milk.

• India is the world's largest producer of milk, with a production of over 210
million tonnes in 2021-22. However, only a small fraction of this milk is forti-
fied with plant-based vitamin D3. As of 2023, only a few milk brands in India
are fortified with plant-based vitamin D3, and the total volume of fortified
milk is estimated to be less than 0.1 million tonnes. https://fortification.fs-

• According to a 2023 study, only 0.1% of milk produced in India is fortified


with plant-based vitamin D3.

• As per the FSSAI standards, milk needs to be fortified with Vitamin A and D
at a level of 270 µg RE - 450 µg RE per litre and 5 µg -7.5 µg per litre, re-
spectively.

Loopholes in Milk Fortification With Plant-based
Vitamin D3 in India

• There are a number of challenges that


need to be addressed in order to increase
the fortification of milk with plant-based vit-
amin D3 in India. These challenges in-
clude:

• Lack of awareness among consumers and


healthcare professionals.
• Lack of government regulations and
standards.
• Lack of quality control and monitoring
mechanisms.
• Potential for adulteration and fraud.
What is Preferred Vit D for
Fortification
Vitamin D: Vitamin D is a fat-soluble vitamin that
plays a crucial role in maintaining bone health, sup-
porting the immune system, and regulating calcium
and phosphorus levels in the body.

 Vitamin D3 is the preferred form of vitamin D


for milk fortification because it is more bioavail-
able, has a longer half-life, and is more effect-
ive at raising 25-hydroxyvitamin D levels.

 However, vitamin D2 is still used for milk forti-


fication in India because it is cheaper and more
available. It is important to raise awareness
among milk processors and consumers about
the benefits of vitamin D3 so that more milk
brands in India can start fortifying their milk with
vitamin D3.

Other Considerations
 Cost of fortification: The National Institute of Nutrition in
India estimated that the cost of fortifying milk with plant-
based vitamin D3 to a level of 500 IU per 1000 ml is approx-
imately INR 0.1 (10 Paisa) per litre. This is a very small cost
relative to the overall cost of milk production and distribution.
 Impact on taste and texture of milk: No Change in taste and
texture of milk as echa gram of plant based Vit D3 powder con-
tains 100000IU/g and which is sufficient for 200 litre of milk

 Acceptance by consumers: it is expected that plant-based vi-


tamin D3 will become the preferred form of vitamin D for
milk fortification in India. This will be due to the many bene-
fits of plant-based vitamin D3, such as its bioavailability,
longer half-life, free from Animal source, cruelty free, sus-
tainable and greater effectiveness at raising 25-hydroxyvita-
min D levels
